Job overview

The Macmillan ‘Improving the Cancer Journey’ (ICJ) Lead will work as part of the dynamic Norfolk and Waveney Integrated Care Board (ICB) Cancer Transformation team. Supporting the drive to improve cancer outcomes and quality of life, by leading on the integration and improvement of the cancer journey across care settings within Norfolk and Waveney.

This fixed term 18-month role will manage the delivery of the system ICJ project within the Norfolk and Waveney Cancer Transformation programme. Leading on the work to promote synergistic working within existing system stakeholders, to include community leaders, councils, services, VCSE (voluntary, community or social enterprise) and patients with the aim of improving outcomes for the Norfolk and Waveney population, by reducing inequalities and fragmented approach to care delivery.

This exciting hybrid role has a work location base at Norfolk County Council offices in Norwich. Stakeholder engagement and attendance on site at least once a week is required, although this can vary depending on meetings, workloads, team engagement and training requirements. The opportunity is available as a secondment for NHS employees and flexible working options will also be considered on the proviso that the expected objectives could be achieved within the 18-month term of the role. We can consider part-time working for this role, however the minimum number of days is 22.5 hours over 3 days.

Main duties of the job

The ICJ Lead will promote synergistic working within existing alliances, community leaders, councils, service users and VCSE (voluntary, community or social enterprise), bringing together services, both commissioned and voluntary to support a patient centric approach to care delivery across Norfolk and Waveney Integrated Care System. With the primary aim of improving outcomes for the Norfolk and Waveney population, by reducing the inequalities and fragmented approach to care delivery.

For this to be achieved by developing networks and having a governance framework to bring together like-minded people together to support seamless delivery of care regardless of which domain the organisation sits within.

To foster trust and erosion of barriers of the organisations an operational framework must be co-designed which will include data sharing agreements to encourage onward referral of patients to the most appropriate resource for them.

Working for our organisation

Health and care services in Norfolk and Waveney are working closely together to further improve services and provide more joined-up care for local people. This is the culmination of many years of effort to build partnership working across the NHS, local authorities, the third sector and patient groups.

 

Detailed job description and main responsibilities

Role specific objectives include:

  • ·         Stakeholder Engagement to scope a place-based non-clinical model for Norfolk and Waveney: To facilitate co-design of the model with stakeholders to include people affected by cancer, carers and communities.
  • ·         To map existing community support and information services in Norfolk and Waveney
  • ·         To review processes for sharing personalised care interventions between primary and secondary care, with the aim to identify and implement any improvements. Some example personalised care interventions are Holistic Needs Assessments (HNAs), Personalised Care and Support Plans and End of Treatment Summaries and Cancer Care Reviews.  Improving how the information is shared will help to avoid duplication, ensure that any issues are followed up between primary and secondary care and so that the patient may only have to tell their story once if appropriate.
  • ·         To review the quality of how Cancer Care Reviews are delivered by primary care and to make recommendations for their improvement.
  • ·         Integrate and awareness of the Core20PLUS5 agenda into the approach.
  • ·         Facilitate stakeholder engagement events to raise awareness of the ICJ model, personalised care interventions, Macmillan templates and cancer training to support the delivery of personalised care in cancer.
  • ·         To scope alternative models if required.
  • ·         To set up and manage a Stakeholder Steering Group to oversee the project and keep stakeholders informed on progress.

  • ·         To undertake an evaluation and report on findings

Knowledge, training and experience

Essential criteria
  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master’s level equivalent.
  • Evidence of post qualifying and continuing professional development.
  • Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement.
  • Should have an appreciation of the relationship between the EOE Cancer Alliance and individual provider and commissioning organisations.
