NHS Norfolk and Waveney Integrated Care Board (ICB) plans and buys healthcare services for our local population. We are accountable for the performance and finances of the NHS across Norfolk and Waveney – a total budget of £2 billion a year. Known as NHS Norfolk and Waveney, the organisation works with local people, health and care professionals, and partner organisations to improve the health and wellbeing of our population.

The organisation is part of the Norfolk and Waveney Integrated Care System. A system dedicated to working with partners in local government, the voluntary sector and others and helping the NHS to support broader social and economic development and to tackle inequalities in health outcomes.

Job overview

·       We are looking for a highly motivated, Senior Practitioner in NHS Continuing Healthcare (CHC) and complex care to join us in our transformation journey. You will support the Associate Director of NHS CHC and Complex Care and Director of Quality Assurance in Complex Care to deliver NHS Continuing Healthcare and Complex Care. This includes commissioning and oversight of care provision commissioned to meet specific needs that are not met through standard commissioned services and drive excellence in the quality and effectiveness of services that meet these specific needs. In addition, you will have excellent organisational and people skills and the ability to work with and influence staff within and outside of the ICB.

 

Main duties of the job

·      

The focus of the role will be:

·       Contribute to the development of the vision, aims and business objectives of the NHS Continuing Healthcare team and wider ICB.

·       Have operational oversight of the delivery of the NHS Continuing Healthcare function including assessments, reviews, commissioning and case management.

·       Ensure clinical oversight of the complex patient group, ensuring that there are plans in place or in development for the safe and appropriate admission and discharge from the service.

·       Oversee monitoring of packages and that the care plans are being delivered as agreed.

·       Provide expert clinical advice and management of this patient group to the ICB and its members.

·       Establish and monitor standards across all commissioned providers regarding patient safety and quality, ensuring that visits and inspections validate and assure compliance take place. Working collaboratively with providers to achieve the best outcomes for our local population.

Essential criteria
  • Educated to Masters level or equivalent level of experience of working at a senior level in specialist areas.
  • Extensive knowledge from a variety of specialist subject areas
  • Evidence of continuing professional development (ahead on future developments in their area of expertise)
  • Must have an understanding of the background to and aims of current healthcare policy in Norfolk and Waveney and appreciate the implications of this on engagement
  • Should have an appreciation of the relationship between NHS England/NHSI and individual provider and commissioning organisations
  • Member of relevant professional body
  • Highly developed communication skills for delivering key messages to a range of stakeholders both internal and external to the organisation, at very senior level.
  • Engaging and comprehensive presentational skills for conveying complex concepts to a varied audience
  • Ability to use informed persuasion to influence others
  • Ability to identify risks, anticipate issues and create strategies which resolve service issues in relation to project or delivery.
  • Ability to understand a broad range of complex information quickly and making decisions where opinions differ/no obvious solution
  • Evidence of planning and delivering programmes and projects and services on time.
  • Experience of controlling financial budgets, resources and staff management
  • Working knowledge of Microsoft Office with intermediate keyboard skills
  • Ability to travel to places of work that may not be accessible by public transport
  • Ability to work without supervision, providing specialist advice to the organisation, working to tight and often changing timescales
  • Interpreting national policy for implementation
