NHS Norfolk and Waveney Integrated Care Board (ICB) plans and buys healthcare services for our local population. We are accountable for the performance and finances of the NHS across Norfolk and Waveney – a total budget of £2 billion a year. Known as NHS Norfolk and Waveney, the organisation works with local people, health and care professionals, and partner organisations to improve the health and wellbeing of our population.

The organisation is part of the Norfolk and Waveney Integrated Care System. A system dedicated to working with partners in local government, the voluntary sector and others and helping the NHS to support broader social and economic development and to tackle inequalities in health outcomes.

Our main offices are based at 8th Floor of Norfolk County Hall, Martineau Lane, Norwich, NR1 2UE however we have a local office for the East of the region based in Beccles and another in the West of the region based in King’s Lynn, therefore travel across the Norfolk and Waveney footprint as well as out of county is required according to where our stakeholders are situated.

This is a new post following the recent restructure of the ICB and the planned retirement of the current Director of Planned Care and Cancer. This post holder will support the ICB Executive Medical Director, and lead the newly restructured Long term condition and Planned Care  Transformation teams, to deliver the ambitions of the Norfolk and Waveney Clinical Strategy and Joint Forward Plan.

The Associate Director will be the subject matter expert for planned care (including cancer) and long term condition clinical transformation across Norfolk and Waveney. They will lead the ICB team and direct various programmes of large scale change, working closely with primary and secondary care colleagues, and other system partners including patient groups, to ensure appropriate the best outcomes for our patients and the best value from the Norfolk and Waveney pound.

 

The AD will also play a key role in strengthening clinical stewardship of planned care and long term condition management, with a focus on enabling system wide clinical transformation, to drive delivery of the ICS clinical strategy and Joint Forward Plan. They will work closely with the Executive Medical Director and Deputy Medical Director to support our ambitions to improve the quality of all of our services, streamline our clinical pathways, and move them closer to the patient wherever appropriate. This is an exciting time to work in clinical transformation in Norfolk and Waveney, as we ready ourselves for the introduction of a single EPR for our three acute trusts, and longer term prepare for the building of two new hospitals as part of the National New hospitals programme. 

 

The post holder will also be available to deputise for the Executive Medical Director across aspects of their portfolio pertaining to planned care and cancer, and will join the ICB Operational (silver) on call rota.

Health and care services in Norfolk and Waveney are working closely together to further improve services and provide more joined-up care for local people. In Norfolk and Waveney, we have already achieved a lot by working in partnership to improve health and care outcomes. These changes have been made possible by different organisations – NHS hospitals, GPs, mental health and community health services, local councils, care homes and social workers, voluntary and community organisations and others – joining forces to agree and plan for local people’s needs.

Norfolk and Waveney Integrated Care System (ICS) includes a statutory Integrated Care Partnership (ICP), and an Integrated Care Board (ICB) called NHS Norfolk and Waveney. This partnership and organisation dedicated to making sure that organisations work together for the benefit of our residents, staff and communities is an important step change, helping to create positive differences to local people and joining up health and social care. This is the culmination of many years of effort to build partnership working across the NHS, local authorities, the third sector and patient groups.

Ensure that the Long Term condition and planned care transformation team contributes directly to the implementation of the ICS Clinical Strategy.

Lead and support the ICB LTC and planned care clinical transformation teams.

Work closely with the performance team to ensure that clinical transformation resources are targeted at areas of greatest need for our system, as well as national priorities.

Work closely with public health colleagues to ensure that public and population health data are used to inform the planning transformation and prioritisation of LTC and planned care services.

 Work closely with the ICB commissioning team, to ensure that services are commissioned in line with that data and the evidence base, to support the transformation plan and input effectively into place-based plans as required.

Lead the development, implementation, delivery and monitoring of the ICS LTC, Planned Care and Cancer Programmes and associated work streams.

 Responsible for strategic transformation and clinical change to support delivery of cancer waiting times constitutional targets

Work closely with the population health management and health inequalities teams to support prevention and reduce health inequalities in particular with respect to the CVD prevent and cancer prevention agendas.

Measure, monitor and report on programme delivery regularly through the Scheduled Care Board and to the Executive Medical Director, and other key stakeholders.

Promote and enhance clinical and care professional leadership through engaging front line clinicians in co-designing future service provision.

Lead projects on behalf of the ICS to facilitate the transformation of LTC, cancer and other planned care, throughout Norfolk & Waveney where appropriate for example women’s health and ageing well.

Working closely with the Deputy Medical Director, support the ICS clinical and care professional leads in driving improvements along entire clinical pathways.

Ensure the securing of value for money, giving due consideration to all relevant factors including risk, quality and other factors.

Ensure appropriate workforce planning and development is built into the design of care models and pathways.

Ensure pro-active implementation of effective people management practices and procedures and development opportunities.

Analyse highly complex information, including performance metrics, financial information, data on local health needs and national/local policies, guidance and legislation to inform key transformation initiatives.

 Explore new ways of developing services, improving quality, managing and controlling risk whilst proactively encouraging innovative approaches and new ways of working in across patient pathways.

Work with clinical colleagues and governance colleagues to ensure that clinical commissioning policies relating to long term condition management, planned care and cancer are effective and equitable, and are applied fairly across the population.

Support the Chair, Chief Executive and Executive Medical Director of Norfolk & Waveney Integrated Care Board (ICB) and other senior colleagues in responding to requests for ad hoc briefings and external engagement activities.
