Job overview

The Diabetes and Endocrinology department is the largest in the East of England, and one of the largest in the UK. The department directorate supports a full range of specialist and general clinical services.

The main Endocrinology & Diabetes dept is based on level 3 in the East Outpatients block. We also have a team working in the hospitals’ Clinical Investigation Unit and a team at Rouen Road, Norwich.

The post holder will be expected to contribute to the provision of the highest standards of patient care.

The Programme Manager, Diabetes Consultant Outreach Service works autonomously, mainly from home, liaising with 7 diabetes consultants and the DFAC (diabetes specialist facilitator team).  Responsible for the Consultant Outreach Service, coordinating meetings and support for 62 general practices in central Norfolk.

The role involves liaison with the chairs of cNIDM (central Norfolk Integrated Diabetes Management Group) to organise and minute the regular meetings.

Work with the chairs of the NHPDF (Norfolk Health Professional Diabetes Forum) to organise training events and meetings for primary care clinicians.
The Programme Manager is supported by an Assistant (B3 7.5 hrs pw) to collate NNUH data for the outreach meetings.

Main duties of the job

  • Provide direct support to GP practices in central Norfolk alongside the Consultants and DFACs.  Provision of practice level data from NNUH Information Services, Diabetes Eye Screening and East of England Ambulance Service to identify high-risk patients
  • Access performance statistics from the National Diabetes Audit, QOF and Eclipse for practices to allow comparisons with other practices within their primary care network, Norfolk and Waveney and England averages.
  • Liaison with GPs and N&WICB commissioners in the development of agreed service implementation plans to support the delivery of NICE guidelines in Central Norfolk.
  • Responsible for the regular review, update and printing of the Diabetes Personal Handbooks.  Organisation of storage of Handbooks and distribution to primary care in N&W, EBDC, NCHC and Norfolk Foodbanks.
  • Provide practices with a list of jointly managed patients (EBDC and DFAC) showing target date, date of next appointment if booked, or appointment status (pt DNA/cancelled/unable to contact).
  • Organise and support the quarterly Central Norfolk Integrated Diabetes Management Group (cNIDM) meetings attended by N&W ICB commissioners, diabetes consultants, diabetes facilitators, GPs, practice nurses and patient representatives.

Detailed job description and main responsibilities

  • To provide management and coordination of the Diabetes Network to ensure that agreed objectives are met in line with local, regional and national targets across primary and secondary care in Central Norfolk.
  • To influence the development and implementation of local policies, protocols and guidelines.
  • To manage diabetes projects.
  • To provide HR and pastoral support to the Diabetes Patient Supporters.
  • Be responsible for aspects of health and safety / incident reporting at Network and Patient Supporters meetings and events.
  • To closely work with a range of stakeholders across the network and the diabetes patient champions to facilitate improvement of diabetes services.
  • To involve other teams in diabetes service development by the use of effective communication and collaboration across organisations and professional boundaries.
  • Ensure the effective involvement of patients, carers and professionals in developing and reviewing services.
  • To be responsible for holding budgets for diabetes education and Diabetes Patient Supporters Programme.  To provide timely budget reports to appropriate bodies.
  • To provide the secretariat to the Diabetes Network, ensuring a robust communication strategy is in place.
  • To plan, organise and support meetings
