NNUH is currently the busiest Trauma Unit in the East of England caring for almost 850 trauma patients annually many with complex care needs. We are enhancing our urgent and emergency care specialist services for patients with significant injuries as a result of trauma and have an ambition to become a Major Trauma Centre.

This part time Band 6 Patient Education Nurse Specialist role within Nephrology is an exciting opportunity for an experienced renal nurse with an interest in providing holistic pre-dialysis care and education to the Chronic Kidney Disease patient group within our community to support the existing Nurse Specialist in this field.

The patients group consists of approx. 300 community based patients, all at different stages of their disease process and all requiring an individualized approach to their pre-dialysis care, management,  and education, ensuring that they are optimized & prepared for Renal Replacement Therapy.

As post holder your duties will include:

  • To operate a care programme for patients with chronic kidney disease, providing education and individualised preparation for End Stage Renal Disease.  
  • Provide information to patients about the treatment options available for management of ESRD.
  • Oversee patient attendance at a ‘One stop’ MDT low clearance outpatient clinic for new CKD patients.
  • Organise and manage a Hepatitis B Vaccination programme for CKD patients.
  • Oversee and manage anaemia in the patient group

  • To manage a specified case load of patients, providing information and support, to enable patients to make informed choices about their care and understand the rational of investigations and treatments.
  • To work closely with the Consultants, medical and nursing staff, multi-disciplinary team and Heads of Department within the Directorate.
  • To ensure that patients are seen within recommended time scales and targets where these are in place.
  • To visit inpatients as appropriate to ensure continuity of care and to give advice/ support.
  • To accept referrals from Nephrology medical/nursing staff  of newly diagnosed and existing patients requiring speciality input.    
  • To periodically review caseload and to discharge patients appropriately within divisional guidelines
  • To communicate sensitive condition related information effectively with patients and carers in a timely and understandable manner, using different communication methods to clarify understanding.
  • To liaise with other Specialists Nurses, Consultants, GPs, Dieticians, other members of the MDT, voluntary agencies, National charities, other agencies within and outside the Trust.
  • To work as an autonomous practitioner responsible for managing and prioritising own workload against the needs of patients and service requirements
  • To assist the Line Manager in the provision of clinical and managerial leadership and support of all nursing and clerical staff
  • To deputise for senior colleagues in their absence as required
