Job overview

We welcome applicants with a wealth of knowledge in acute and or critical care nursing whom are seeking to develop their senior nurse leadership skills. The CCOT team here at HDFT are developing service, whom are well supported by the PSC directorate and Multidisciplinary team members.

The Critical Care Outreach Lead Nurse will be a clinical expert and professional resource to the Critical Care Outreach Team and provide extensive clinical advice and support to ward-based staff. 

The post holder will contribute to the development of the specialist area of practice in conjunction with other members of the speciality team (lead clinician, lead nurses for critical care, business manager etc.) and will work collaboratively to develop services and quality of care delivered.

The post holder will manage and lead the team of specialist practitioners.

Main duties of the job

Critical Care Outreach Team (CCOT):

 The aim of this team is to ensure quality care to all acutely ill adult patients at Harrogate and District NHS Foundation Trust, using highly skilled nursing, medical and support staff. This is achieved by working closely with the parent and/or on-call teams in conjunction with the wider MDT. Clinical Supervision and Leadership will be provided by the Matron and the Clinical Lead for the CCOT. The CCOT also provides education to nursing and medical staff within the organisation in a variety of areas and participates in local/regional audit to shape practices at HDFT.

Key working relationships:

To work with patients and family/carers to provide specialist nursing knowledge, care and support for the acutely ill adult patient in the ward environment across HDFT.

The post holder will be supported by the Critical Care  and Theatres Matron and the ICU Consultant Clinical Lead for Outreach and by ICU Consultants on Outreach sessions.

To provide cross cover and support for the Critical Care Unit and staff in the absence of their Lead Nurse.

To work in conjunction with all staff within the MDT to ensure high standards of patient care.

To forge positive working relationships with other staff within the Trust and regionally via the Critical Care Network in particular the Critical Care Network Forum.

  • Degree level education or equivalent experience and knowledge
