The Acute Response and Rehabilitation in the Community and Hospital (ARCH) have recently secured significant additional resource, which will allow the service to expand the cohort of patients who can be identified to leave hospital sooner with additional short-term support in their home environment reducing the need for inpatient rehabilitation, risk of deconditioning and interim placements

Our ARCH rotation provides the opportunity for successful applicants to rotate between core areas of the service, to develop competencies that support positive patient outcomes, 7 days a week.  The rotations across key areas include- virtual beds, community rehabilitation, emergency department, urgent community response, admission avoidance service and and hospital in reach. This provides varied opportunities across the hospital base and community and you will get the chance to work in all of the areas.

Successful candidates will be expected to partake a 7 day service covering 8am to 8pm across all rotational areas.

Main duties of the job

To deliver rehabilitation services within a multidisciplinary team to inpatients and community patients. Working closely with other professionals to ensure positive clinical outcomes and effectiveness by providing a co-ordinated service including continuity and quality of care.

To support patient flow from hospital to their own homes by giving appropriate information, advice and education.

ARCH is a friendly and supportive team focussed on improving the quality, safety and experience of patients and also the development and support of the staff within the team.

Person specification

Essential

Essential criteria
  • Full UK driving licence and access to a car
  • Relevant NVQ Level 2/3 or equivalent
  • Relevant health/ social care experience in frailty
Desirable criteria
  • Experience of rehabilitation/ reablement/ promotion of patient independence
  • Knowledge of home first/ D2A
  • Understanding of medicines management
