Job overview

York and Scarborough Teaching Hospitals NHS Foundation Trust have an exciting opportunity to join our Community Integrated Care services as a District Nurse Community Team Leader.

We are seeking a dynamic, forward thinking, credible, clinical District Nursing Team Leader, to offer steer and support to one of our District Nursing Teams within the West Locality.

As well as the day-to-day operational and clinical management of the team, the post holder will have a focus on achieving high quality care and improving services. There will also be opportunities to work with the wider Management Team to develop collaborative working with stakeholders from health and social care.

The successful candidate will have access to a robust induction programme this will include leadership development the post will also provide further opportunities for personal and professional development.

Main duties of the job

We are looking for a District Nurse who possesses experience of working within the community setting, you will have a broad set of clinical skills and the ability to provide supportive leadership and operational management to the wider community nurse team. These roles will require you to have overall responsibility for the caseloads within WEST to ensure high quality, efficient and safe care is provided.

You will report into the respective Community Matron who will support and guide you through regular 1-1 meetings. This role will also give you experience of working with our partners within the wider Integrated Care System.

We are looking for caring, understanding, enthusiastic and flexible leaders who possesses the clinical, leadership and digital skills to promote quality improvement for a local population.

Education, Qualifications and Training

Essential criteria
  • RN
  • Post registration Qualification (Specialist Practitioner District Nurse qualification)
  • Degree or equivalent experience
  • Teaching and Assessing qualification
Desirable criteria
  • Relevant post graduate qualification or management qualification
  • Post registration Leadership course

Experience & Knowledge

Essential criteria
  • Recent experience managing a team
  • Experience of working across agency boundaries
  • Experience teaching and assessing students, support staff and peers
Desirable criteria
  • Experience of clinical benchmarking i.e. Essence of Care
  • Experience of clinical audit and performance management
