We are looking for a dynamic and highly motivated senior clinical pharmacist with a interest in caring for frailty patients to join our pharmacy frailty team.

 

Working alongside another senior colleague and a junior team you will be responsible for providing a specialised level of pharmaceutical care to frailty patients in the acute hospital, rehabilitation, and the novel enhanced care at home settings.

 

Using your prescribing qualification, you will provide expert advice on prescribing and de-prescribing in relation to inappropriate polypharmacy, falls prevention and adherence to medication.

 

You will also integrate into the Frailty MDT at our York Hospital site. This role involves assessment and treatment of patients in an array of settings including ward round and clinic style consultations. Experience of effective engagement with patients and team members is a vital skill for this post.

You will work closely with the senior pharmacist for Frailty at York Hospital to implement systems to improve the safe transfer of care for frailty patients. You will also work with the senior pharmacist for Frailty at Scarborough hospital to review systems of practice cross-site. You will work alongside directorate leads to investigate incidents and promote medication safety and deprescribing initiatives. You will also develop close links with primary care colleagues.

You must possess excellent communication skills, have strong clinical skills and must demonstrate competence in technology and project management. We welcome interest from pharmacists from all backgrounds, but you must hold a postgraduate qualification in clinical pharmacy to Diploma level and be (or be willing to train to be) an independent prescriber. 

This post includes participation in working the weekend (1 in 8), bank holiday (1 per year) and on-call rotas. On-call working frequency (from home) is only approximately 1 in 30.

  • Degree in pharmacy (Masters or equivalent)
  • Registered member of the General Pharmaceutical Council (GPhC)
  • Continuous Professional Development to meet the requirements of the GPhC
  • Postgraduate qualification in pharmacy at least to Diploma/MSc level, relevant to post or equivalent experience

  • Member of Royal Pharmaceutical Society of Great Britain or specialist pharmacist group
  • Management training
  • Non-Medical Prescribing qualification

Experience

  • Broad post registration experience at least part of which must be of hospital pharmacy A length of experience sufficient to gain the competencies required for the post
  • Knowledge of Medicines & Pharmacy Legislation, professional ethics, Health & Safety and the wider Health Service
  • Knowledge of Clinical Governance, Medicines Optimisation, Patient Safety, Risk Management, NICE guidance, Shared Care Guidelines, and the development agenda relevant to the post
  • Knowledge of theoretical and practical issues, including professional guidelines and legislation, related to use of medicines and practice of pharmacy in the specialist areas
  • Understanding and practical experience of audit and service improvement
