Renal Dietitian

Job overview

You will predominantly be working with CKD outpatients and you will also spend some time working within Kettering General Hospital to provide a service to renal inpatients. The acute part of the role is 0.48wte. The remaining 0.3wte is to provide a service to dialysis patients who receive care at the Kettering dialysis unit. The post offers a good variety of clinical work and an excellent opportunity to work in an established MDT.

Main duties of the job

·        Telephone clinics for advanced kidney care, some clinics may be attended in person and off main site.

·        Providing in-patient care for dialysis patients and advanced kidney care patients who may require specialist nutrition advice

• Undertaking comprehensive assessment of patients with complex presentations, multipathologies within acute inpatient and outpatient settings and provide effective monitoring.

• Formulating individual management and treatment plans for patients using advanced clinical reasoning and developing comprehensive discharge plans including advice on appropriate ACBS prescribable enteral feeds and supplements  to be used. This includes secondary prevention

• Working collaboratively with the wider multi- disciplinary health care team to ensure delivery of coordinated care and to provide specialist advice and attend case conferences, network meetings as appropriate.

• Evaluating patients’ progress and measuring outcomes and contribute to and implement service improvement work

• Using a range of verbal and non-verbal communication tools to communicate effectively with patients to implement treatment plans. This will include patients who have difficulty with understanding communication

• Providing advice and training to patients, carers, colleagues and other healthcare professionals to promote an understanding of the aims of dietetic treatment to ensure a consistent approach to patient care.
