Band 4 Pathway navigator - Liver Surveillance

Job overview

·         The purpose of this role is to continue the implementation and maintenance of a liver cirrhosis database to enable the hepatology service to continue to deliver exceptional patient care and meet minimal national standards, as outlined in the “NHS England Hepatocellular carcinoma (HCC) surveillance: minimum standards” guidelines.

·         Form strong professional relationships with other clinical teams and partner agencies and work closely with them to enrol all new and existing eligible patients from the gastroenterology service.

·         Monitoring, and maintaining the database of those on Hepatocellular Carcinoma surveillance

·         They will support the patient through the journey from identification and referral to conclusion of their intervention, thus eliminating barriers to care.  

·         The pathway navigator will work closely with the wider gastroenterology MDT to ensure a patient focussed service and will keep accurate patient and activity records to facilitate reporting back to Commissioners regarding performance and service success.  

·         To evaluate surveillance data, to ensure the hepatology service meets national standards of care and provide regular data submissions to NHS England and the East Midlands Cancer Alliance.

·         Support the nursing team and to encourage individuals living with, and at risk of, Liver Disease, but who are not yet engaged in treatment, access healthcare services.

Main duties of the job

·         To ensure the trust meets national minimum standards of Hepatocellular Carcinoma (HCC) surveillance as outlined in the NHS England Hepatocellular Carcinoma (HCC) surveillance: Minimum standards.

·         Monitoring & Managing the HCC surveillance database ensuring it is up to date.

·         Co ordinate patient’s abdominal ultrasound scans and blood tests and ensuring results are evaluated by a clinical nurse specialist.

·         Liaise with different departments within the trust and external to the trust to ensure patient care is met.

·         To help reduce patient cancellations and non-attendances for tests and clinic appointments influencing rapid surveillance pathway

·         To undertake a range of administrative duties which facilitate and support coordination of the surveillance programme including assisting in arrangement of appointments, tests.

·         To escalate issues with the surveillance programme in a timely fashion

·         To actively engage in service improvement  to maximise patient experience and outcomes.

·         To participate in local and national audits

Person specification

Educations, Training and Qualifications

Essential criteria
  • Educated to GCSE 3 level of education, training or experience
  • Computer literate with good working knowledge of Microsoft packages
  • Experience of using spreadsheets to a high level
Desirable criteria
  • IT qualifications – eg MOST, ECDL
  • Experience of using databases

Knowledge and Experience

Essential criteria
  • Knowledge of a range of administration duties and procedures
  • Excellent communication skills both written and verbal
  • Courteous, tactful and efficient telephone manner at all times
  • Proficient in Microsoft packages including Word, Excel & Outlook
  • Experience of database systems
