This is an exciting opportunity to become part of the Child Health Directorate at the Northampton General Hospital. You will join a creative multi-disciplinary team that aims to shape healthcare for the Children of Northamptonshire as the team adapts to the evolving needs within Child Health and the growing population of Northamptonshire.
This is a locum Acute Paediatric Post which has arisen due to the retirement of one of our longstanding paediatric colleagues. The locum will be for a year in the first instance with a view to converting to a substantive post at the end of the locum.
The post offers an exciting opportunity to become part of the Child Health Directorate at Northampton General Hospital. You will join a creative, dynamic, forward thinking, multi-disciplinary team that aims to shape healthcare for the Children of Northamptonshire as the team adapts to the evolving needs within Child Health and the growing population of Northamptonshire. We are constantly looking for innovative ways of working. The post holder will have the opportunity to further develop the Paediatric Epilepsy Service alongside having wide General Paediatric Experience +/- other special interests.
The post holder will also be encouraged and supported to develop services in collaboration with the nearby trust in the current collaborative working model under the University Hospitals of Northamptonshire.

General:
- Deliver high standard of care by working closely with medical and nursing colleagues and other supporting staff
- Develop clinical pathways by collaborating with colleagues
- Manage a flexible approach to service provision and the needs of the service
Clinical:
- Participate in a consultant of the week and on-call rota on the general paediatric wards and Paediatric Assessment Unit (PAU).
- Support junior doctors and nursing staff
- Hold outpatient clinics
- Take over the care of an existing out-patient cohort. Amongst this cohort are C&YP with Epilepsy and associated problems.
- Possibly develop an additional or wider specialist interest in Neurology, to compliment the department and become the lead in that role.
Clinical Governance:
- Participate in Epilepsy 12 and other clinical audit projects
- Participate in Trust and Department’s Quality Improvement Programmes
- Undertake the clinical risk management and clinical governance activities of the department and Trust
Continuing Professional Development:
- Participate in CPD activities as required by the Royal College of Paediatrics and Child Health
- Be involved in departmental educational activities
- Be involved in annual appraisal process as required by GMC
Teaching and Training: - If you enjoy teaching, there is plenty of opportunities to develop this role including working with our excellent simulation centre.
- Contribute to the teaching of junior staff and medical students
- Contribute to the training of nursing staff

A formal job plan and timetable will be agreed between the new Post Holder and the Clinical Director, and will be effective from the commencement date of the appointment.
The post holder will join the Consultant of the Week and Paediatric Admissions Unit rotas
Consultant of the Week:
This involves covering the Paediatric Wards, Monday to Friday 0830 to 1700hrs. You are present at the 08:30 handover from the night team, and the 16:30 handover to the evening team. During those weeks the post holder is not expected to do routine clinics.
Paediatric Admissions Unit:
This involves covering the Paediatric Admissions Unit, Monday to Friday, 09:00 – 17:00. The Consultant carries the admissions bleep whilst covering PAU. During those weeks the post holder is not expected to do routine clinics.
General Paediatric and Epilepsy Service Duties
The initial timetable (when the post holder is not on Consultant of the Week rota) is as follows:

Epilepsy Service Duties
- · Member of the Paediatric Epilepsy Team working alongside Dr Bolu Akinbiyi (lead) and Paediatric Epilepsy Specialist Nursing Service within and outside outpatients.
- · To ensure service remains compliant with NICE Guidelines.
- · To see children promptly in outpatients who have a convincing history of epilepsy or those who move into the area with a preexisting diagnosis of epilepsy.
- · Be able to provide advice to colleagues looking after children (inpatients or outpatients) who have or might have epilepsy.
- · Willingness to take over the care of children with Epilepsy who fail on first line medication and / or have “difficult to treat” Epilepsy.
- · Develop closer working with Clinical Psychology.
- · Provide Epilepsy-related education to medical students, doctors in training and to nursing and medical colleagues. Organise Paediatric EEG meeting (twice per year)
- · Ensure local guideline remain up to date.
- · To maintain participation in Epilepsy 12 Programme and share findings of Annual Report with the Child Health Department, Trust Audit Department and Trust Management (where applicable).
- · Be able to provide specific advice surrounding the use of Sodium Valproate in children and young people to colleagues, parents, and patients. To counsel and complete MHRA paperwork surrounding the use of Sodium Valproate in children and young people, including providing a second opinion and countersigning documentation. To maintain local register of those children and young people on Sodium Valproate
- · Develop standard and personalised care plans for those requiring emergency treatment
- · Help prepare young people with epilepsy for Transition and attend Transition Clinics. Work closely with NGH Adult Epilepsy Team to ensure consistency of care
- · Work closely with Oxford Regional Paediatric Neurologist (currently Dr Rohini Rattihalli) in care of patients with Complex Epilepsy and ensure prompt access of suitable patients to the CESS (Children’s Epilepsy Surgery Service) pathway.
- · Remain up to date with BPNA PET training & attend PET4ward course every 5 years
- · Participate in 6 monthly Regional Network / ODN meetings (Oxford Regional Epilepsy Networking Group – ORENG)
- · Participate in weekly Epilepsy MDT meeting – this would include service development as well discussions of patients with epilepsy not necessarily under the care of the Epilepsy Team. Patients discussions may cover social and psychological issues as well as medical issues. It may also involve viewing videos of events in patients uploaded onto the IslaCare system
On call Commitments
The appointee will join 1:8 rota for the paediatric on call team for general paediatrics – there is a separate neonatal rota.
There are both Tier 1 and Tier 2 doctors on site 24 hours a day. The consultant on-site availability varies between winter and summer on-calls as below:
Winter weekday on-calls
From 1st October – 1st March the post holder will not be expected to be on-site until 16:00 for the afternoon handover (cancelling clinical commitments on on-call days). They are expected to be on-site from 16:00 – 22:30. They are then able to complete their on-call off-site if the clinical situation allows
Summer weekday on-calls
From 1st March – 1st October the post holder will work a normal working day and then attend the afternoon handover. Following this, they are able to complete their on-call off-site if the clinical situation allows
Weekend on-calls
Weekend days are split, with one person undertaking Friday and Sunday and a separate person undertaking Saturday on-call. The on-call will be 24 hours 08:30 – 08:30. The post holder will attend the morning handover at 08:30 and then conduct the ward round on both wards. They can then complete the on-call off-site if the clinical situation allows.

- Entry on the Specialist Register or be an SpR or Trust Grade Doctor within 6 months of CCST / CESR date
- Level 3 Safeguarding training
- APLS provider status or equivalent
- PET1-3 training

- Experience working independently at Consultant Level
- Instructor for PALS/APLS
- SPIN, MSc, BPNA distance learning course in Paediatric Epilepsy
Clinical Experience

- Wide range of paediatric and neonatal experience at middle grade level and above (including UK tertiary level experience)
- Experience of working in the UK
- Ability to offer expert clinical opinion on and manage a range of problems in the acute paediatric setting
- Up to date knowledge of the outpatient management of common paediatric conditions Ability to take full and independent responsibility for clinical care of patients in ward, emergency department and outpatient setting
- Ability to deal with complex clinical situations in the core areas of the post
- Experience of working in a Paediatric Epilepsy clinic including initiating anti-seizure medication
