Home Oxygen Clinical Nurse Specialist

An exciting opportunity has arisen for a Home Oxygen Clinical Nurse Specialist to join our Respiratory Therapy Acute REsponse Team (RESTART). 

Working across primary and secondary care the Home Oxygen Service Nurse Specialist will act as the clinical expert and professional resource to the multi-professional team and patients and carers within the specialist service. The post holder will work alongside the Restart team and other healthcare professionals within the speciality to ensure the development and delivery of a high-quality service for patients requiring oxygen 
assessment review and follow up. This will include the management of a caseload of patients, and the provision of specialist advice within the clinical area. The role will include the provision of ward-based assessments, advice and management of patient discharges with oxygen, clinics, home visits and support and education for existing services. The post holder will act as a clinical leader and role model for the ongoing development of clinical practice, research and standards of care. Effective liaison and 
good communication skills are essential to the post.

To manage a caseload of patients with oxygen needs, delivering individualised and personalised direct patient care.
To take and correctly interpret capillary blood gases and prescribe oxygen appropriately following national guidelines.
To manage Point of Care testing of all equipment according to local and national requirements.
To provide clinical expertise and specialist advice within a defined area of practice
To support, encourage and advise clinical teams, by the provision of specialist knowledge and advise in the care of patients within the speciality.
To participate in the promotion of patient centred care, establishing and maintaining a supportive relationship with the patient and their families during the period of treatment and care.
To support and enable patients and carers to make informed decisions relating to their treatment and management.
To promote high quality clinical care, and assist in the clinical development of the specialist service, by ensuring the use of evidence-based practice and clinical standards within the speciality.
To provide professional support and clinical guidance to nursing staff within the speciality as required 
To provide professional support and clinical guidance to hospital clinical staff around safe discharge and risk assessment of inpatients for Home Oxygen

Demonstrating appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of patient services. This will necessitate high levels of autonomy and advocacy.
To acknowledge limitations and when necessary, make referral to chest physician particularly if diagnosis is in question
Acting as a role model, demonstrating high standards of care and providing clinical leadership in the specialist area.
Maintaining a high standard of verbal and written communication to provide continuity of care and ensure that this is evident in clinical teams
Policy/Service Development Implementation
To develop a comprehensive oxygen service alongside other members of the multidisciplinary team to meet Department of Health and British Thoracic Society recommendations
Establish primary care diagnostic and assessment clinics that will improve diagnostic accuracy in patients with oxygen requirements in the community.
To develop and influence policy making, contributing to the development of policies and clinical guidelines both internally and externally, necessary to in support of the specialist service
To promote a culture of innovation and change to meet the needs of the service
Participating in key developments relating to the speciality, so that high quality patient focused services are promoted

Education, Training and Qualifications

  • Degree or significant respiratory experience at this level
  • NMC Registered Nurse

  • Capable of studying at master’s level
  • Non-medical prescriber

Knowledge and Experience

  • Credible clinical background in a relevant acute/community respiratory speciality
  • Experience of managing resources
  • Experience of managing change
  • Able to facilitate, plan and deliver a range of respiratory education and training for professionals, patients and carers
  • Able to understand and implement research in practice
  • Able to effectively utilise evidence based practice ie, standards, audit, clinical risk assessment, protocols
