Job overview

We seek dynamic nurses with amazing interpersonal skills to support the leads in the county wide Community Rapid Response & Intermediate Care Team. We are looking to recruit new team members who can proactively and effectively deliver a range of treatments, including IV therapy and all aspects of nursing care in the patient’s own home environment.

The Community Rapid Response & Intermediate Care Team operates as a Virtual Ward across the county, managing patients with sub-acute conditions to aid their recovery from illness or injury within their own homes by preventing unnecessary hospital admissions and reducing length of hospital stay.

In order to respond to the healthcare needs of the local population, the teams operate 7 days a week ensuring the delivery of the service can respond to patients’ needs as and when required. The shifts for the service are between 07.00am to 01.00am including weekend and bank holiday working as part of the rota. The successful candidates will be expected to work within these shift patterns. We can offer long and short shifts.

 

Main duties of the job

We are looking for experienced RN’s with a passion for high quality nursing care and also experience in either acute medicine, Emergency Care or Community Nursing. The post holder will provide acute nursing assessment & interventions in patients’ own homes and will ensure that patients are referred onwards as appropriate to community services once their care episode is complete. The vision of the service is the key to bridging the gap between hospital and community - bringing care closer to home and we also provide dedicated nurses into the front doors of both our acute hospitals in the county.

  • Clinical triage skills
  • High level assessment, diagnostic and nursing clinical skills

Education/Qualifications

Essential criteria
  • Professional qualification
  • State Registered Nurse
  • Evidence of CPD including attendance at recent postgraduate learning events relevant to the clinical field of Intermediate Care
  • Portfolio evidence of learning

Knowledge/Understanding

Essential criteria
  • Up to date with professional practice and research
  • Understanding of clinical governance and its implication for services including quality and audit
  • Understanding of legal responsibilities
  • Ability to comprehend and work within the Trusts policies
  • The ability to deal with difficult emotional and social circumstances, including patients who have been given poor prognosis
Desirable criteria
  • Membership of Interest Groups relevant to clinical interest
  • Data analysis, identifies and uses appropriate analytical methods to ensure the successful analysis of data

Experience

Essential criteria
  • Specialist knowledge underpinned by broad experience in a wide range of specialisms to including Orthopaedics, Medical, Out-patients, older persons and community over a period of years
  • Experience of working with a range of standardised tools to underpin nursing practice and demonstrate outcomes
  • Working in a multi disciplinary setting
  • Of being a student Practice Educator
  • Recent NHS experience
  • Relevant experience of working at Band 6 or equivalent level
Desirable criteria
  • Experience of Intermediate Care setting
  • Previous experience of electronic patients notes systems
