Job overview

Northamptonshire Healthcare Foundation Trust has a fantastic opportunity for a Nurse Practitioner to come on board to help develop an up-and-coming Virtual Clinical Care Team.


This service is being formed to monitor patients at home, or in care homes, to ensure clinical safety netting and support to detect early deterioration.
The service uses innovative technology and other NHFT services to ensure patients in Northamptonshire receive the necessary treatment to support our Home First ethos.


We are seeking a dynamic, flexible, and experienced Nurse Practitioner with experience across a range of health conditions, including acute, long-term frail and elderly, who will be able to support the on-going development of our Community Team. The post holder will be confident in providing effective clinical leadership to the team and work well under pressure. They must possess advanced clinical assessment skills, be a confident prescriber, able to problem solve and think on their feet.

Main duties of the job

The successful candidate would also be required to lead and co-ordinate compassionate, high quality and evidence-based care by supporting the Clinical Lead and working in partnership with other System Partners, including the Northamptonshire ICB, to ensure the safe and effective delivery of the service throughout its operational times .

You will need to provide strong leadership, possess excellent communication skills and be able to develop and support staff with clinical and professional expertise.

  • Registered Nurse or Paramedic

  • Experience in project management and or project management qualification
