Job overview

Mid Notts Place Based Partnership brings together health and social care services across Mansfield, Ashfield, Newark and Sherwood. NHS providers, commissioners and local authorities are working together across Mid-Nottinghamshire to improve the lives of the residents in our communities. This partnership has been built around six ‘Primary Care Networks’ (known as PCNs). A PCN is a collaboration of GP practices working alongside other services in the community. Each PCN is directed by a Clinical Director who works in that area.

This role is key to integrating our wider partners covering District Councils, Voluntary & Community Sectors and many more as part of our place delivery plan. The role is extremely varied and will require a successful candidate to support PCNSs, to connect our system partners to work collaboratively, to deliver key elements of the ICS and ICB Strategies, develop Integrated Neighbourhood Working, build and maintain relationships, to support the resilience of General Practice and to understand and employ a Population Health Management approach towards transformation.

The successful candidate should have an in-depth knowledge and appreciation for current issues and challenges in health and social care and have experience and knowledge of working in across the sectors of Primary Care, Community Care, Local District Councils, Social Care and voluntary sector organisations.

Main duties of the job

·       Support continuous improvements in health and wellbeing services at Primary Care Network (PCN) and Place level, delivering transformation projects that support prevention (primary, secondary and tertiary) and integration.

·       Provide support across a range of areas to enable Place and PCN partners including Clinical Directors, PCN members to work collaboratively, develop strategies and share best practice across PCNs and Place

·       Work with wider community partners to drive system changes to deliver key elements of the Integrated Care System (ICS) Strategy / Joint Forward Plan / Place Plan and Primary Care Strategy

·       Work collaboratively with PCNs, Community and wider partners to develop integrated neighbourhood working.

·       Build relationships with partner organisations to support transformation within PCNs and Place including (but not limited to) acute, mental health, community health providers, voluntary sector and community members.

·       Provide project management leadership and / or support for Place / PCN transformation projects.

·       Help facilitate collaborative working to improve the resilience of general practice, support local community services and aid the recruitment and retention of key members of the primary care workforce.

·       Employ a Population Health Management approach to transformation.

Detailed job description and main responsibilities

 

·       Assist the Place Director and Deputy Directors (Place Transformation) to ensure that the ICB and Place Team exercises its functions safely, effectively, efficiently and economically. 

·       Work constructively and collaboratively with other members of the team to ensure that statutory duties are met and that objectives are met.

·       Support delivery of the Primary Care Network, Place Partnership and ICB objectives.

·       Engage clinicians, GP practices, patients and other stakeholders in proposed service changes and help to promote new pathways of care.

·       Ensure transparency and accountability through the commissioning process.

·       Undertake project support and research, review data for the development of new and existing services and pathways where appropriate.

·       Implement effective processes and procedure around process analysis, service design, resource planning and financial management of a designated portfolio of work.

·       Contribute to the development of place specific plans to enhance primary care and community provision to support delivery of care closer to home.

·       Support the development of plans that contribute to health and social care integration in order to provide seamless services for local people.

·       Contribute to the development and implementation of plans relating to population health management, reducing health inequalities, using the Building Blocks of Health and supporting proactive care.
