By leading a team of dedicated link workers and collaborating with various stakeholders, you will contribute to the development of a holistic and person-centred approach to healthcare within Bulwell & Top Valley  PCN.

As a PCN Social Prescribing Link Worker Manager, you will play a vital role in overseeing and coordinating the social prescribing initiatives within a Primary Care Network (PCN). You will lead a team of social prescribing link workers and work closely with healthcare professionals, community organizations, and other stakeholders to support the well-being and health outcomes of patients.

Main duties of the job

  • Provide effective leadership to a team of social prescribing link workers, including recruitment, training, and performance management.
  • Foster a positive and collaborative team culture, encouraging innovation, creativity, and professional growth.
  • Support link workers in their day-to-day activities, providing guidance and supervision as needed.
  • Develop and implement a comprehensive social prescribing program in collaboration with healthcare professionals and community partners.
  • Establish and maintain strong relationships with healthcare providers, community organisations, and local authorities to promote the benefits of social prescribing.
  • Collaborate with PCN leadership and other managers to integrate social prescribing into the wider healthcare strategy.
  • Act as a liaison between the PCN, link workers, and external stakeholders, facilitating effective communication and partnership working.
  • Ensure adherence to relevant policies, procedures, and guidelines in the delivery of social prescribing services.
  • Monitor the quality and safety of interventions, identifying and addressing any concerns or risks promptly.
  • Stay updated with the latest developments in social prescribing and related fields, incorporating best practices into the program.
  • Establish data collection and reporting mechanisms to monitor outcomes and demonstrate the impact of social prescribing interventions.

Education & Professional Qualifications

Essential criteria
  • Level 4, advanced level, equivalent qualifications or relevant experience.
  • 5 years experience in Management role.
Desirable criteria
  • Level 3 Certificate in Social Prescribing
  • Completed E-Learning For Healthcare - Social Prescribing
  • Project management qualification or equivalent experience
  • knowledge of key NHS stakeholders including NHS England individual provider and commissioning organisations

Experience

Essential criteria
  • Proven track record of effective team management, including setting clear objectives, providing regular feedback, and fostering a collaborative work environment.
  • At least 5 years of experience in managing a team of 5 or more employees, including hiring, training, performance management, and career development.
  • Experience of working collaboratively and inclusively with diverse communities and with the principles of Asset Based Community Development.
Desirable criteria
  • Experience of working in a Healthcare setting
  • Experience of healthcare systems such as SystmOne and EMIS
  • Previous experience working as a Social Prescribing Link Worker

Essential criteria
  • Working knowledge of Microsoft Office including high level of expertise in MS Excel
  • The postholder will be required to work at various sites in the PCN. The postholder may need to work between sites during the working day which would require frequent use of transportation and mobility between sites carrying relevant resources.
  • Experience in promoting diversity and inclusion initiatives and managing a multicultural team

Desirable criteria
  • Sound working knowledge of SystmOne and EMIS
