Job overview

Nottingham City Council has secured funding to deliver a pilot programme for Active Travel Social Prescribing (ATSP) to explore how social prescribing can support people to do more walking and cycling. 

The Active Travel Care Co-Ordinators will work in three areas of the City to support the role out of the ATSP programme. The Active Travel Care Co-Ordinators will be the main point of contact for GPs and patients about the walking and cycling support offer funded by the ATSP programme in each project area.

The Active Travel Care Co-Ordinators will offer support within general practice and various locations within the community including the patient’s home. The role will provide information and support to patients in addition to becoming the link between the patient, GP and other service providers.

The role will work closely with colleagues in the City Council and NCVS as part of cross-sector ATSP project team requiring the post holder to manage and prioritise their own caseload, in accordance with the needs, priorities and any urgent support required by individuals.

Main duties of the job

The role aims to improve understanding of “what matters to me” for our target groups to support behaviour change to active travel for people previously not engaged or unable to take part, due to health, confidence, mental wellbeing, mobility, or other physical or financial barriers.

The walking and cycling activities provided by the ATSP programme will focus on supporting:

·       Level 1: People at risk of low healthy life expectancy through family-friendly walking and cycling activities in the community to support parents/carers with young children to become more active.

 

·       Level 2: People experiencing low mental health or social isolation through one to one and small group activities such as walk and cycle buddies, cycle training and group walks and rides to build confidence and overcome social isolation.

  Level 3: Patients attending Nottingham University Hospital’s clinics with non-alcoholic fatty liver disease and for people with gestational diabetes through walk and talk clinics to make active lifestyle choices to help to manage their condition and improve their health and wellbeing.

·       Level 4: Active travel taster sessions for GPs and social prescribers to build understanding of the benefits of walking and cycling.

Detailed job description and main responsibilities

The Active Travel Community Connector will support patients into Level 1 activities and more intensive and personalised Level 2 activities including providing support with the associated participant monitoring. The role will also be an advocate and champion for walking and cycling within the PCN workforce to support patients to take up the Level 4 activities.

Person specification

Experience

Essential criteria
  • Experience of working within a sport, health, care, wellbeing service either in the NHS, VCSE or statutory sectors
  • Experience of providing one to one motivational support to individuals to support lifestyle changes
  • Role model for active travel behaviour through regular walking or cycling for leisure and/or transport
Desirable criteria
  • Previous experience of leading or managing a team.
  • Experience of providing one to one, group, family-friendly walking and/or cycling or other activities in the community

Education

Essential criteria
  • NVQ Level 3, advanced level or equivalent qualifications or working towards or relevant work experience
Desirable criteria
  • Level 3 Certificate in Personal Training Motivational/sport coaching and interviewing skills qualification or equivalent experience.
