An exciting opportunity has arisen at the Wolfson Centre and its 17 bed ward for a qualified social worker to join the Adult Cystic Fibrosis team. This a full time role, with the centre opening Monday-Friday, from 8am-5pm. Support and training will be provided, there is also an opportunity for flexible working and home working.
This is a fantastic opportunity to further develop your social work skills within an NHS setting alongside an established and supportive MDT.

The practitioner will respond to the psychosocial aspects of care relating to patients , their carers’ and family members. The service will be provided to a large geographical area, including Nottinghamshire, Derbyshire and Lincolnshire. It is based at the Wolfson CF Centre ward and outpatient clinic. Home visits will be an important aspect to follow up needs in the community.
The practitioner will support the Adult Cystic Fibrosis Regional Service in all aspects social/psychological needs. The post holder will work in close liaison with the relevant social services department and other professionals in line with relevant legislation, professional standards and departmental policies.
Working in partnership with the multi-professional team will be a fundamental requirement to ensure that social aspects of the patient, family and carer needs are promoted and addressed.

Detailed job description and main responsibilities
As CF Social Worker you would be responsible for a caseload of patients needing support and advocacy; also providing support to carers, siblings and other family members. The successful candidate will work in collaboration with our wider multi-disciplinary team members providing both co-working, support, guidance and supervision. An aptitude for assessment and care planning is a key element of the role as well as advocating for our patients. Excellent communication skills are pivotal. The successful applicant will demonstrate an enthusiasm to become part of the CF Team with a willingness to learn and develop further personally whilst also contributing to service developments along the way.

- UK recognised qualification in social work eg BA Social work, CQSW, Diploma in Social Work
- Significant experience of social work – able to work independently
- Evidence of CPD
