Discharge Support Officers

Are you interested in being part of a busy team and helping to develop the  Discharge support officer role and Discharge to assess processes in NUH?  

We are looking for a motivated and enthusiastic individual who is  committed to supporting the discharge process and are passionate about improving patient care.  Are you currently working as a Band 3 or 4 in a patient facing role and ready for a new challenge?

The Integrated Discharge Team (IDT) have an exciting opportunity for a Discharge Support Officer (DSO) Band 4 role in the team to support our admin and clinical teams in the development of Discharge to Assess and hub process. 

If you have experience of working with patients, an awareness of discharge processes, knowledge of using Nerve centre and want to be part of a forward thinking team at the start of this exciting new development this could be the role for you. 

We have been piloting this role as a new addition to the Discharge team and due to the role's success we are now able to offer these permanent post.
