Are you passionate about improving neonatal care and driving positive change within healthcare services? Nottingham University Hospitals NHS Trust is seeking a dedicated and dynamic individual to join our team as a Transitional Care Service Improvement Lead on a 12-month secondment. This is a unique opportunity to play a pivotal role in enhancing our transitional care services to meet the BAPM transitional care standards. You will have the chance to influence the future of neonatal care, working closely with a team committed to delivering exceptional outcomes for newborns and their families.

Our maternity and neonatal redesign project aims to create a seamless, integrated care pathway that supports babies and their families from hospital to home. We are focused on developing innovative models of care that improve health outcomes and patient experiences. By joining our team, you will contribute to shaping this transformative initiative, ensuring that our services meet the highest standards of care and support.

Prif ddyletswyddau'r swydd

  • Drive change and identify service improvement needs.
  • Provide clinical care to babies on the transitional care ward when needed.
  • Collaborate with the lead nurse, transitional care consultant, and maternity partners.
  • Develop and implement action plans and a new model of care.
  • Represent neonatal transitional care at meetings and prepare reports.
  • Network with other transitional care units in the East Midlands Neonatal Operational Delivery Network.
  • Ensure smooth transition from hospital to home for eligible babies.

  • RNC or RNA or RM
  • Neonatal QIS (qualified in speciality)
  • Degree in nursing related studies
  • Master’s degree or equivalent or willingness to work towards/equivalent experience.

Experience

  • Current or recent experience of working in a neonatal or Transitional care unit
  • Education and training experience
  • Significant post registration experience at band 6 or above, in a neonatal and/or Transitional care setting
  • Demonstrates professional autonomy and effective decision making skills when managing caseload
  • Experience in supervision of registered, non-registered staff and students
  • Ability to manage clinical area/ward efficiently and effectively
  • Ability to convey sensitive info in an empathetic manner to patients, relatives and staff
  • Experience in using Badgernet, DHR and NOTIS
  • Experience of leading audit or research projects within the clinical area
  • Leadership and management experience, including providing staff with feedback on their individual performance and/or conducting appraisals
  • Experience of managing risk, handling clinical incidents and complaints
  • Demonstrates awareness of research and evidence Based practice.
  • Evidence of teaching and assessment skills, including the teaching of patients, families and colleagues
  • Evidence of working with a multidisciplinary team in appropriate setting (ie hospital, community)
  • Experience of developing standards, guidelines and policies
  • Understanding of neonatal and maternity patient pathways and it’s complexities and how to support patients through this

  • Demonstrate basic IT skills
  • Can minimise interpersonal conflict and barriers to communication, demonstrating evidence of negotiating and influencing skills
  • Ability to work within a MDT; collaborating with, leading when required, negotiating with and influencing the work of others across a range of disciplines and on own initiative including the sharing of highly complex information and implementation of plans for Service development.
  • Ability to deliver presentations and chair meetings
  • Ability to support colleagues, patients and relatives/carers
  • Experience of initiating and implementing change effectively, engaging stakeholders in the process.
