There is an exciting opportunity within the Neonatal Home Care Team. We are seeking dynamic, enthusiastic and motivated Band 5 Nurses to be part of our Home Care team helping to provide a comprehensive 7 day service.  

The neonatal home care team is based at both campus of Nottingham University Hospitals NHS Trust (NUH) and aims to promote safe early discharges of babies from the Neonatal units and Transitional Care.   

Being a part of the Neonatal service at NUH means you will also work within the north hub region of the EMNODN. This will allow for opportunities to network across the north hub, access training and support and gain knowledge in other neonatal services within the network. This may also involve travel across the region. NUH is also undergoing a wonderful change with the Maternity Neonatal Redesign (MNR) which will see Neonatal services grow significantly over at QMC.

These posts are permanent and will support early repatriation of the family, create education packages for parents and staff and provide surgical support.   These posts are specialist nursing roles, and will enrich your career by providing you with skills to carry out nursing neonates in different environments.   You will touch the hearts of families, as they will yours by building relationships, helping them through their experience providing emotional support. 

Main duties of the job

Candidates must have experience of caring for babies in a hospital or community environment preferably with additional training relevant to the role. This post offers a fantastic opportunity to utilise and develop your existing knowledge and skills in caring for babies in the home setting, enhancing future career progression.

These posts will support the existing home care team in planning the care and transition from the neonatal unit and transitional care wards through a structured discharge process, promoting safe early discharge from hospital to home. The post will also support the discharge of babies for short term home phototherapy from postnatal and children’s wards. You will liaise with other members of the multidisciplinary team and support parents in providing high quality care in the home environment.

You will see the families’ confidence grow and empower them to care for their baby from admission to discharge, to the journey home.   It is a unique experience to be able to continue give the family back the decision making of the baby’s care, with your support.

The service is run over a 7 day period, currently between the hours of 8-1700, however, this will change to 08:00-19:00 when home phototherapy is rolled out.  Successful candidates will be expected to work these hours as a combination of short and long shifts once a period of induction has been completed.

As part of the Neonatal Service at NUH, the post holder will actively support families in the care of their baby (who fulfil the stated criteria in the Neonatal Service Home Visiting Guidelines and Neonatal Home Phototherapy Guidelines) both on the neonatal units and transitional care areas.

As part of the Neonatal Service at NUH, the post holder will assure that all babies (who fulfil the stated criteria in the Neonatal Service Home Visiting Guidelines) receive access to the home visiting programme after discharge from the neonatal service.

The post holder will participate in the discussion around discharge planning with the parents/carers of the baby, the medical team, low dependency team and wider multidisciplinary team prior to discharge of the baby and then facilitate the documented home care plan for this baby at home until such time that the baby is deemed fit to transfer care to the relevant health visiting team and wider community.    

The post holder will act to ensure that the standards and quality of care given to babies, together with the environment in which care is delivered, are maintained at a high standard.  The post holder will be expected to facilitate and co-ordinate home care and family centred care.

Candidates will be expected to undertake a regular clinical shift on either unit, equivalent to 10% of their weekly hours.

Communication and relationship skills

Essential criteria
  • Demonstrate effective written and verbal communication skills
  • Evidence of use of electronic communication systems, intranet, internet
  • Produce timely, accurate written/electronic records and documents
  • Demonstrate basic IT skills
  • Recognition of own level of competence and limitations and able to communicate these to appropriate staff
  • Can minimise interpersonal conflict and barriers to communication

Training and Qualifications

Essential criteria
  • Current Level 1 NMC Registration
  • Experience of working with neonates in a ward or community setting
  • Working towards Post registration Neonatal Qualification or a qualification in paediatric community/health visiting
  • Newborn Life Support provider (NLS)

Planning and organisation skills

Essential criteria
  • Demonstrate an ability to prioritise own workload and recognise, prioritise and respond appropriately to urgent and emergency situations
  • Demonstrate effective time management skills

Experience

Essential criteria
  • Knowledge and skills demonstrating a breadth of understanding of neonatal care
  • Ability to convey complex and sensitive information in an empathetic manner to parents, relative/carers and staff
  • Able to work with an identified caseload competently and with confidence recognising the needs of the multidisciplinary team
  • Able to work independently and make decisions safely
  • Personal development plan identified

Analytical and Judgement skills

Essential criteria
  • Demonstrate an ability to assess, plan, deliver, prioritise and evaluate patient care
  • Ability to identify own learning needs
  • Able to respond to problem situations and ensure that effective interventions are put in place

Physical Skills

Essential criteria
  • Physically able to perform the full range of nursing duties
  • Dexterity and accuracy in undertaking clinical skills, use of equipment and documentation
  • Competent and capable of using medical equipment
  • Able to undertake patient manual handling manoeuvres and non-patient manual handling manoeuvres
