Job overview

An exciting opportunity has arisen for 2 experienced Cancer Support workers to join Cancer Clinical Nurse Specialist Teams to help support patients and deliver the Personalised Care agenda to our patients for a 24 month fixed term basis.

Each post is  Macmillan funded for 24 months and will work in one of the following cancer CNS teams in the Oncology and  Gynaecology  specialty.  The post will  be based at City Hospital site but there may be a requirement to support at the QMC also. 

Each post will be embedded within Cancer Clinical Nurse Specialist Teams and work closely with each MDT to offer and support patients in the completion of Holistic Needs Assessments and the development of a support plan, as well as to support in the implementation of treatment summaries and a sustained personalised stratified follow up pathway if appropriate.

We are seeking experienced, enthusiastic and highly motivated individuals with excellent communication,  interpersonal and organisational skills to work in this innovative role, demonstrating how the unique role of the Cancer support Worker can support both our patients and our Clinical teams.

The Macmillan Cancer Support worker  role will work as an integral part of Cancer Nurse Specialist Teams to support patients living with Cancer and assist with delivering the personalised care offer for all patients.

 The Cancer support worker will support the cancer nurse specialist team by having appropriate regular supported conversations with the person living with cancer, assisting in distributing Holistic Needs Assessment tools for patients to complete. They will support the CNS in providing relevant information and advice including signposting the patient to other Macmillan resources and services supporting them to self-manage.

The post holder will support the Cancer Nurse Specialist Team in delivering the core principles of the Personalised Care in Cancer services, specifically the ‘Macmillan Right by You Principles’. They will  assist the CNS to coordinate care with the person living with cancer so they can access the right help at the right time, providing a seamless experience of care. 

The overriding purpose is to support the provision of the highest quality patient/client care through personal actions and continuous improvement.

To provide  supportive  care for a designated group of patients/clients during a care episode within cancer care services.

The post holder will promote a team nursing philosophy of care.

Detailed job description and main responsibilities

Supports the cancer nurse specialist team by having appropriate regular supported conversations with the person living with cancer, assisting in distributing Holistic Needs Assessment tools for patients to complete. Supports the CNS in providing relevant information and advice and signposting the patient to other Macmillan resources and services supporting them to self-manage. 

Following a holistic needs assessment, assists the CNS to coordinate care with the person living with cancer so they can access the right help at the right time, providing a seamless experience of care. Checks that the person is able to self-manage and provides a flexible and personalised response to any changes in need.

Navigates the complex health and social care system, both during and following completion of cancer treatment, by building relationships with professionals and facilitating access to these services including supporting the CNS to generate referrals on the person’s behalf where necessary.

Supports the CNS to provide practical and emotional support to the person living with cancer and their families. Recognises information needs may change over time and works with the person to ensure they have all the information and support they need at all stages of their cancer journey.

Supports the CNS to document and monitor all aspects of care coordination and service delivery, supporting data collection on ‘Macmillan Right by You’ acute Performance Metrics, and how people living with cancer use the service, including care, support and follow up services they access.

To work alongside partners in the community, hospitals and care homes to gather information for the purposes of completing assessments and review.

Work with other team members to provide support and advice to patients and carers in relation to holistic  needs.

Assist with the delivery and signposting of patients to Cancer Health and well being pre and post treatment programmes

To maintain accurate, clear, concise and contemporaneous records and communicate information to members of the team as necessary.

To liaise with external agencies with regards to the development and implementation of care packages.

To contribute to the development, delivery and evaluation of service improvements and quality initiatives.

Guided by organisational and professional policies.

Ensure own mandatory training is kept up to date, including clinical supervision.

Put people first and provide nursing care that is person centred, safe and compassionate

Training and qualifications

Essential criteria
  • • Educated to A-level, and or NVQ Level 3 or 4 in Health and or Social Care
  • • Evidence of Continued professional development
Desirable criteria
  • • Registered Nursing Associate on the NMC register

Experience

Essential criteria
  • • Experience of working in a multidisciplinary team
  • • Experience of working in a healthcare setting particularly with patients with cancer
  • • Knowledge of patients’ needs when being treated / cared for in a cancer setting
  • • Demonstrate an understanding or have experience of Personalised care in cancer including Holistic needs assessment. Experience of multi agency working.
  • • Ability to demonstrate empathy
Desirable criteria
  • • Good knowledge of speciality patient pathways

Communication and relationship skills

Essential criteria
  • • Demonstrate effective written and verbal communication skills.
  • • Ability to communicate effectively with people at all levels, communicating sensitive information to service users and internal colleagues
  • • Experience of using Microsoft Office, including Word, Excel and PowerPoint Experience of using Microsoft Office, including Word, Excel and PowerPoint
Desirable criteria
  • • Communication skills training (eg Macmillan Sage and Thyme)
