Highly Specialist Speech & Language Therapist

We are excited to offer an opportunity to join the SLT service within Nottinghamshire Healthcare Foundation Trust. We work with adults with Intellectual and Developmental Disability in inpatient and community services. This is an opportunity to support people to progress towards discharge and to live well in the community. Your focus will be communication, mental wellbeing, and dysphagia support.

 

37.5 hours. We welcome applications for the full-time position or from people who are interested in one of the posts only. 

 

Community with dysphagia specialism  22.5 hrs: You will play a key role in co-ordinating and delivering highly specialist SLT input to the community intellectual disability team. The post involves working with adults with dysphagia and communication needs and offers a mixed caseload. You will be based in the Mansfield/ Ashfield area with opportunities for working from home.

 

 

Inpatients 15 hrs: You will be leading the specialist SLT team in an inpatient assessment & treatment service for adults with intellectual disabilities, as part of an established MDT. This is an exciting opening to provide comprehensive communication and dysphagia support. In line with the principles of Transforming Care this is a great opportunity to make a difference to the lives of those in an inpatient setting, working with them, their families and support networks to plan for successful discharge from the outset. You will be based at the hospital for the duration of these hours.

Your role will be to support the person to live well with their mental health and eating/ drinking needs.

In the inpatient service, you will be working as a member of the Multidisciplinary team to develop a thorough understanding of the patients’ communication needs. You will be collaborating on supporting the person to progress towards discharge and community living. You will be supporting the team with your specialist communication knowledge and your understanding of the individual’s needs based on the assessments and observation you will be undertaking.

 

In the CIDT, you will be working in the local community and be supporting the person’s carers to adapt their environment to meet their eating, drinking and communication needs. Our overall purpose is to support the individual and their carer to maintain positive relationships to avoid placement breakdown and admission to hospital.

 

In both roles, you will be a member of a multidisciplinary team and you will be instrumental in the holistic formulation of the individual’s needs and care plans by adding a functional and environmental communication perspective to the discussion (using the 5 Good Communication Standards as one of the principles of inclusive communication).

Specialist speech, language, and communication assessments focusing on functional communication and what reasonable adjustments (Accessible Information Standards) should be in place to support the person’s needs.

Liaison, advice and recommendations that aim to prevent further communication breakdown and that aim to empower the person to express their thoughts and opinions to the extent they wishes. You will work in an interactive and a consultative way. 

The role will also involve assessing and developing the management of eating and drinking difficulties for individual’s, where issues arise due to a variety of physiological, cognitive and environmental reasons.

Co-working with your MDT colleagues, families, and carers to enable their inclusive communication practices in their relationships, care, and interactions with the individual.

Attendance at meetings with and on behalf of Orion Ward and CIDT and the individual to plan for ongoing and accessible care.

Training

  • Portfolio of relevant post qualification CPD including highly specialist knowledge of Speech, Language and Communication Needs (SLCN) of service users with Aspergers, Autism, Developmental Language Disorder, mental health diagnoses and other neuro-developmental disabilities

  • Post-graduate training specific to client group – autistic people with ID with criminal offences

Experience

  • Working experience of assessing and managing a complex caseload with young people and/or adults with speech, language and communication needs.
  • Experience of effective multi-disciplinary/agency working and established interpersonal and negotiation skills
  • Experience of designing and delivering different learning and development opportunities for staff teams.
  • Experience of delivering environmental approaches to SLT service delivery and evaluating them

  • Highly specialist experience of working with autistic people with autism, complex intellectual disabilities and mental health diagnoses/personality disorders
  • Experience of linking communication to risk issues

Knowledge
