Discharge Co-Ordinator

Job overview

The post holder will support the Transfer of Care Hub in Bassetlaw working closely with the community therapy team in Bassetlaw supporting pathway 1  and pathway 2 ( Step down/ up beds) whilst  being supportive to Adult Community services across Bassetlaw  county and other services if required. The post holder will be responsible for the effective coordination of patient discharges across the acute and step down bed based services, to optimise patient flow and bed capacity. They will work closely with clinicians, social workers, carers, third sector – Age UK and all multidisciplinary team members to deliver a high quality patient focussed service. They will meet the required standard to proactively overcome identified concerns /issues that may delay effective discharge from services. They will coordinate with clinicians in the MDT to co-ordinate assessments of patients’ needs and home circumstances and support the formation of the discharge plan.

Main duties of the job

The role requires the Discharge Co-Ordinator to have organisational skills to manage their own workload, and skills to support the Hub team in managing their patients’ journey through the service and through to an effective and timely discharge. The individual will need to be highly motivated and have effective interpersonal skills; able to maintain the delivery of a high quality service during the change process. The post holder will develop innovative new ways of working with other providers to enhance improved working practices and relationship across healthcare and contribute to the delivery of the  Discharge 2 Assess  agenda. The post holder will possess skills that enable them to accurately assess and refer on to other third party services as the patient requires to support a home first approach wherever possible. The post holder will have effective communication skills to communicate with the Hub team as a whole to ensure that all relevant information is accurately shared and communicated across parties.

The post will help to implement total mobile for the Bassetlaw therapy team and will help with coordination of total mobile to ensure patients visits are completed as required.

Detailed job description and main responsibilities

The Bassetlaw community therapy team is a very supportive and caring team. We work closely with the Urgent community team and social care to ensure our patients get the best care in Bassetlaw.  We have a dynamic and adaptable approach to our work, to meet the needs of the patient and the wider system. We require someone who is dynamic able to question why there may be delays in the system, propose alternatives. This is an exciting and diverse opportunity for the right candidate.  Candidate may be required to attend meetings/ training at other sites on a regular basis.

The post holder should have good IT skills, a flexible approach, able to challenge appropriately .   if you enjoy working in a team, at pace as well as being able to impact positively  on patient care and the experience they and families and cares have of our service then we want to hear from you.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Please note that this post does not meet the pay level required for a Skilled worker visa. 

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.
