Job overview

An exciting opportunity has arisen to be part of developing the delivery of CT(E)R's across the hospital, ensuring that all of those with a Learning Disability and/or Autism are identified and supported to engage with the process. This is a brand-new role that has been identified to ensure that those supported by Mental Health services have awareness of and access to both C(E)TR's. The aim of the C(E)TR is to bring a person-centred and individualised approach to ensuring that the care and treatment and differing support needs of the person and their families are met, and that barriers to progress are challenged, overcome, and analysed, options compared and reviewed to agree the best way forward.

This post will be hosted within the David Wilson Unit and will be in-bedded into the teams as the first port of call for all C(e)TR queries.

Main duties of the job

This role requires significant organisational skills, initiative, time management skills and personal credibility.  The role requires an Administrator with the ability to provide an excellent, confidential and efficient service to support the Hospital Transforming Care Programme on a daily basis.

Detailed job description and main responsibilities

To set up and manage information systems applying some level of analysis against agreed criteria and assists in the preparation of reports and present findings clearly to a range of audiences.  Chase progress where information is lacking.

Multi-task and plan work for short; medium and long term timescales.  Plan and organise sometimes complex activities requiring adjustments to be made e.g., resolving conflicting appointments and arrange meetings with a number of participants with complex diaries.

To ensure the efficient day-to-day coordination of services between patients/carers, MDT members, internal and external agencies for the purpose of managing the coordination of Transforming Care Programme Reviews (CTRs)

To contribute to the development, implementation, and evaluation of processes and procedures, negotiating alterations according to changing local, national and organisational needs.

Liaise directly with outside agencies e.g., commissioners, senior managers, Provider Collaboratives, identifying and establishing links with the correct individuals for each patient’s care.

To ensure action plans resulting from external reviews are received and disseminated appropriately.  Maintaining a database of all outstanding actions, reporting progress regularly.

Produce high quality reports withing agreed deadlines as required. 

Be the first point of contact for matters relating to Transforming Care by telephone, email or correspondence.

Providing information to staff and other agencies both internal and external in relation to the patients Transforming Care Review 

To provide administrative support, using appropriate software packages, working to strict timescales and to a very high standard of accuracy, following correct organisational standards. 

Required to prioritise own workload, working effectively using own initiative and working without direct supervision.

Maintain high levels of confidentiality when dealing with sensitive patient information.

Responsible for ordering stock and non-stock items as required, managing purchase orders, blanket orders, and good receipt notices.

 Responsible for own Essential Training requirements

To undertake any other duties that would be a reasonable expectation of the role.

Please note that this post does not meet the pay level required for a Skilled worker visa.

  • Possession of a degree or equivalent experience
  • Knowledge of full range of Business and Administration procedures acquired through NVQ level 3 or above and/or proven experience
