This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust.  This is payable in 3 instalments over a 2 year period.  This payment is only eligible to new starters who do not already hold a contract of employment with the Trust.  This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.

In your new role you will work closely with Physiotherapists, Assistant practitioners,  support staff,  district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.

We are working towards a 7 day service - 8am to 8pm on rota basis. Self rostering is encouraged.

Main duties of the job

An exciting opportunity has arisen to join our developing  integrated community services  providing clinical triage, rehabilitation, falls management, palliative care support and  support timely discharges from our acute Trust partners.

You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient’s independence, safety and quality of life.

The post will involve cross site working and community visits.  Part of the role will be in the South Nottinghamshire Transfer of Care HUB - on a rota basis with B6 colleagues.   A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
