You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient’s independence, safety and quality of life.

The post will involve cross site working and community visits.  Part of the role will be in the South Nottinghamshire Transfer of Care HUB - on a rota basis with B6 colleagues.   A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
