This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.
This is an exciting opportunity for an experienced Mental Health Nurse to join the Bassetlaw LMHT as part of the community transformation programme, in line with the longer-term plan for community services. The community transformation includes opportunities to develop skills within SMI pathways (mood affective, complex psychosis, community rehabilitation and non-psychosis including the Cluster 8 and Trauma Pathway).
We are particularly interested in hearing from people with skills and experience, working with adults with a range of mental health difficulties who require complex case management/clinical interventions.
You will have opportunity to access training, and to develop your existing skills in working within all the relevant LMHT Pathways. We are a very supportive and experienced team who work to a recovery and psychologically informed model.

The post holder will be expected to work with patients and their families to support triage of referrals, assessment, personalised care planning and in the provision of specialist interventions. You will promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.
This is a leadership role, and you will be expected to develop specialist skills in working within all pathways within the team. You will also be expected to support students and more junior staff within the team through clinical supervision and mentorship, and you may be asked to line manage some staff.
We are seeking enthusiastic team players, to work within a dynamic multi-disciplinary health team, with excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies.

Day to Day Duties:
Caseload management for patients living in the community, and acting as care coordinator.
Triage of referrals, provision of initial assessments and one-to-one interventions with service users.
To undertake and record comprehensive need/risk assessments, devise and implement treatment plans for service users.
To demonstrate the application of specialist skills and knowledge in order to maintain professional competence and fitness to practice as a nurse practitioner.
To establish and maintain robust communication networks with clients, carers, team members, other health care and social care workers within urgent care and the community
To use interpersonal skills in diffusing potentially emotive situations while undertaking assessments and pathway planning within acute settings.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
