LDA Hospital at Home is an innovative new service being developed under the Thames Valley Provider Collaborative. The Hospital at Home team will focus on young people with moderate to severe learning disabilities AND/OR a severe degree of functional impairment associated with an autism diagnosis who are at risk of home or placement breakdown and who could meet the threshold for inpatient care.
In some cases, the team will help prevent unnecessary admissions and in others, young people will be stepped down from tier 4 care through the LDA Hospital at Home team, shortening their inpatient admission. LDA Hospital at Home maximizes the use of new technology, provides care close to home, reduces admission, length of stay, waiting times and provides earlier discharge from inpatient care.
By managing the care pathway, we will be able to ensure that children and young people are cared for in an inpatient setting or via an ‘out of hospital’ service as close to home as possible. This will:
- Reduce the burden on families to travel long distances
- Enhance continued engagement with community clinical teams
- Encourage early therapeutic home leave
- Minimise Length of Stay
- Avoid admission
Main duties of the job
As the CAMHS community nurse in the team, you will work as part of the multi-disciplinary team to assess, plan, implement and evaluate the specialist health care needs of children and young people who have a learning disability and/ or Autism.
The team will have a phased implementation.
During Phase 1; the senior team will be offering consultations to the network and contributing to individual cases. They will be preparing for the training and preparation for the rest of the team.
During Phase 2; you will be part of discussions for planned implementation of agreed interventions, on a small number of agreed pilot cases. You will be part of the team to put the agreed interventions in place, working collaboratively with the young person, family and other relevant professionals in the network for that individual. You will have an identified supervisor and attend both 1-1 and group supervision/ debriefs for these complex cases.
During Phase 3: You will continue to be part of the MDT as the team expands, continuing to contribute to the team process' and development.

In this role, you'll be working Mon - Fri, 9am to 5pm, but we also understand the important of a work life balance; so if you can't work full time, we welcome applications from you if you can commit a minimum of 22.5 hours a week.
LDA Hospital at Home is a specialist service working with patients up to 18 years old with moderate to severe learning disability and autism. The Thames Valley Provider Collaborative is a network of service providers covering Oxfordshire, Buckinghamshire, Berkshire, Gloucestershire, Swindon, Wiltshire and Bath and North East Somerset (BaNES), working collaboratively to provide high quality care for the most unwell young people as close to home as possible.
We are currently recruiting our multi-disciplinary team (MDT) who will work with young people diagnosed with moderate to severe learning disabilities and autism, who are at risk of admission due to mental health difficulties.
This is an exciting time to be part of the development of a new Tier 4 service for CAMHS. The LDA H@H team will have close links and working relationships across the Thames valley collaborative area to support existing provision across the patch. The main functions of the team are to prevent admission and facilitate earlier discharge.
You will be based in Oxford in our new offices located above the Meadow Unit on the Warneford site.

- You will have the opportunity to be part of developing an innovative LDA hospital@ home team, with the focus of supporting young people to stay at home with clear packages of care around them.
- We are an inclusive and forward-thinking team, there will be a strong focus on training, CPD, supervision and staff wellbeing.
- This role could be full time or part time (minimal 22.5 hours) with some flexibilities.
- Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required
Person specification
Knowledge
Essential criteria
- Knowledge of physical and mental health conditions affecting people with learning disabilities and/or Autism, and how these are managed.
- Working knowledge of the Mental Capacity Act.
- Understand and have experience of Risk Management including within the identified client group for the LDA H@H team.
Desirable criteria
- Awareness of DOLS
- Awareness of LEAPS and CETR’s
Qualifications
Essential criteria
- Registered Learning Disabilities Nurse or RN Adult or RN Mental Health or RN Child with evidence of knowledge and experience of supporting the health needs of people with learning disabilities and/ or Autism.
Desirable criteria
- Additional qualifications in evidenced based therapeutic interventions.
- Mentor course completed or willing to complete
Further training
Essential criteria
- Ability to complete comprehensive nursing assessments and devise nursing interventions.
- Ability to communicate effectively with people who have difficulty understanding.
- Ability to work under indirect supervision.
Desirable criteria
- • Awareness of a variety of communication techniques

- Able to contribute to team discussion and be respectful of others’ views.
- Aware of personal and professional boundaries
- Experience of working with people with learning disability and/ or Autism from previous employment, training placements or voluntary work.
