We have an exciting opportunity for a Mental Health Practitioner to join an award-winning, sector-leading team that is improving the health and housing outcomes of people experiencing multiple exclusion homelessness and disadvantages.

The Oxfordshire Health and Homelessness Inclusion Team (OHHIT) is an integrated partnership operating across housing, health, care and third sector systems.

Assess mental health with a view to enabling clients to access appropriate primary or secondary care services. They will also assess and work with those clients who will not traditionally meet secondary service thresholds.

·     Act as a keyworker where appropriate for a small group of complex patients for short periods and will also co work with patients who are already established within primary and secondary mental health services where appropriate. This may involve work around housing, benefits, medication and employment and support ongoing communication between services. 

·     Provide initial service for clients with a range of mental health problems until secondary services are in place, arranging speedy transfer to community mental health teams as appropriate.

·     Provide a consultative service for colleagues in primary and secondary health services, and all associated system partners, identifying local resources available in Oxford which will aid recovery of service users.

·     Be peripatetic - proactively outreaching to those sleeping rough for whom mental health concerns have been raised and taking the service to meet identified need in any setting within the various local 'pathways'.
