An exciting opportunity has arisen for highly motivated individual to join our Community Nursing Team covering the Central locality of in the provision of planned care to patients in the community.

We are looking for a caring, motivated and friendly Community Nurse to join our team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care.

Main duties of the job

You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development. We also actively encourage newly qualified nurses to join our team. In return we offer a preceptorship programme and support professional development in a nurturing environment, allowing you to reach your full potential.

The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.

We are committed to clinical support, CPD & training. The post is covering 7 days, 8am to 6pm service.

Qualifications

Essential criteria
  • RGN NMC Registered
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes

Experience

Essential criteria
  • Post registration general experience
  • Experience of working in a multi disciplinary / multi agency health/social care environment.
  • Experience and knowledge of basic administration systems including Microsoft office
  • Experience of successful partnership working arrangements with other services
  • Previous community experience

Skills

  • I T skills or willingness to undertake RIO Training
  • Up to date with Mandatory Training
  • Uses evidence based practice
  • Works within own defined competencies
