An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service.  The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.

We are looking for a Team Lead who shares our passion for providing excellent quality of care and can help devise & deliver a recommended and agreed treatment/care plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.

The post holder will work closely with the Urgent Community Response (UCR) and Virtual Ward Multidisciplinary Teams to provide clinical responsibility for a defined delegate patient caseload, and managerial support for the locality team. They will ensure the delivery of high quality, responsive, efficient and effective management of patients in their usual place of residence, to enable them to remain in their home at a time of crisis, or as an alternative to admission. The post holder will undertake the line management of all personnel within the team.

Training and Qualifications

  • Level 1 Adult Nurse Registration with Nursing and Midwifery Council (NMC) or Degree in Allied Health Profession with relevant registration to Health & Care Professions Council.
  • Qualification in management studies or similar, to diploma level e.g. Leadership in Clinical Excellence or equivalent experience.
  • Evidence of ongoing professional/management development.

Experience

  • Significant post-registration experience and caseload management within community setting.
  • Experience of working in a multi-disciplinary/multi agency health/social care environment.
  • Successful track record as an effective leader/team manager in a health care environment.
  • Experience of building successful partnership working arrangements with other services.

Knowledge & Skills

  • High level of understanding of current professional issues
  • Line management experience and knowledge of HR policies and procedures
  • Good knowledge and understanding of community and primary care services, integrated care pathways, clinical guidelines and Governance and risk management acquired through training and proven experience.
  • Good working knowledge of safeguarding requirements and standards for clinical care within the relevant area.
