Job overview

An exciting Band 6 opportunity has arisen in the Renal Transplant Specialist Team at The Shrewsbury and Telford Hospitals. This is a permanent ,19.5 hour per week post. 

We are seeking to appoint a highly motivated and dynamic Band 6 Specialist Nurse in renal transplantation. 

You will need to have recent renal experience and an eagerness to learn a new role. 

The successful candidate will be able to demonstrate excellent communication skills, team working skills and will be dedicated to improving the patient's experience. You will need to be able to demonstrate good performance and be able to work to tight time constraints on a regular basis.

Main duties of the job

The post holder willl provide a service for patients prior to renal transplantation. They will be responsible for timely listing of patients on the cadaveric transplant list and work with the pre dialysis team to improve pre-emptive renal transplant numbers. The post holder will address the specific needs of these patients, and work collaboratively with the Consultant Nephrologists and the Transplant co-ordinators at Birmingham.

They will be a competent practitioner, leading innovation and demonstrating clinical expertise. 

They will  identify potential donors for assessment for renal transplantation and improve live donor kidney transplant numbers.

Liase with patients and relatives regarding the process for contacting either the transplant nurses at Shrewsbury and Telford Hospitals or the Transplant co-ordinators and Transplant team at Birmingham.

To update and maintain the transplant database on a regular basis. Take responsibility for performing regular audits of transplant service and ensuring the Renal Association Recommendations are met.

Be instrumental in establishing, implementing and maintaining evidence-based care pathways to facilitate effective cadaveric transplant listing and live donor transplant work-up.

To develop nurse led pre transplantation clinics and ensure completion of pre transplant clinical assessment.

To be willing to undertake the Physical assessment Course and Nurse Prescribing if not already completed.

The Renal Specialist Transplant nursing team at SATH are responsible for educating renal patients and families about their transplant options, coordinating their transplant assessment and listing them on the national kidney transplant list in a timely manner. This includes working with the CKD nurses at both Shrewsbury and Telford Hospital. We also work closely with Shrewsbury, Hollinswood House and Ludlow Hospital dialysis units, and our Home Therapies team which include home haemodialysis and peritoneal dialysis patients. 

We have patients who are listed for kidney transplant or kidney and pancreas transplant and currently deal with seven transplant centres in total. We also do Phase 1 tests for living kidney donors in our area and in the future we will develop yearly review for living kidney donors in our catchment area.
