Job overview

An exciting opportunity has arisen to work within the Community Respiratory Service as a Specialist Respiratory Nurse.

The post holder will participate in supporting the developing community respiratory service and deliver comprehensive, skilled and evidence based respiratory care to patients in the primary care setting.

You will support the delivery of proactive patient care, appropriate admission avoidance and facilitate early discharge, fostering close working relationships with Community Nursing, Primary Care, Social care and the Acute Trust.

You will work within an established clinical team who deliver Respiratory care across Shropshire, Telford & Wrekin providing in reach to the Royal Shrewsbury Hospital and Princess Royal Hospital working within an MDT, as we are extremely fortunate to have Pulmonary Rehab/Respiratory Physiotherapy within our Community Respiratory Service. It is a very exciting time to be part of SCHT as we review and expand our community services in conjunction with Shropshire Telford and Wrekin ICS.

Main duties of the job

  • To be a competent practitioner, demonstrating respiratory clinical expertise providing specialist advice and developing training programme for the multidisciplinary team.
  • To assist in the formulation and delivery of patient and 
    carer education programmes to optimise self management.
  • To deputise for the Respiratory Team Lead.
  • To undertake comprehensive assessments and examination of patients including those who may have complex presentations or multiple pathologies, using advanced assessment techniques, adapting and prescribing treatment programmes according to patient need.
  • To participate in the instigation of multidisciplinary and team meetings to discuss ongoing management of individual respiratory patients, particularly those with complex needs.
  • To ensure patients have evidenced based care, continuously evaluating patient progress and self management plans.

Qualifications and Training

Essential criteria
  • Registered General Nurse
  • Nurse Prescriber (or willing to undertake training)
  • Evidence of recent training/ qualification in respiratory care eg diploma in respiratory care or COPD.

Experience

Essential criteria
  • Recent experience in a senior position in respiratory medicine
  • Community experience (desirable)
  • Expedience in treating patients who are end of life

Knowledge and Skills

Essential criteria
  • Extensive knowledge of chronic respiratory conditions and holistic nursing needs of this patient group
  • Advanced clinical skills including physical assessment
  • Knowledge of the COPD and asthma outcomes strategy, NICE & BTS guidelines
  • Ability to plan, implement and evaluate clinical care provided to patients with complex respiratory needs
  • Have knowledge of the End of Life Strategy for COPD
  • Clinical knowledge & skills in respiratory care
