As a frailty advanced clinical practitioner you will work across various areas of the hospital including the award winning GEMS service based in ED, OPAU and frailty SDEC.  You will perform highly advanced assessments of the frail older patient, with diverse presentations and highly complex physical and psychological conditions, utilising a comprehensive geriatric assessment, to determine a diagnosis, formulate and deliver an appropriate, individualised treatment programme.  You will also act as a clinical and professional lead within acute frailty and geriatric medicine among a multidisciplinary team.  You will act as a lead contributor to the development of the acute frailty service as a whole.  You will also have an expert working knowledge of community services to avoid admission and facilitate discharge while also continuing to build key stakeholder relationships to further improve patient pathways, outcomes and experiences.  A trainee opportunity is also available for candidates who do not fully meet the essential criteria.  This trainee opportunity will include an MSc apprenticeship where appropriate and progress to Band 8a when qualified.

  • To undertake a comprehensive, holistic assessment of patients including those with highly complex presentations/multi pathologies, using highly advanced clinical reasoning, analytical and investigative skills and highly developed specialist manual assessment techniques to provide an accurate diagnosis. 
  •  Have an expert working knowledge of the comprehensive geriatric assessment.
  • Actively lead the development of care planning and admission avoidance initiatives through advanced clinical skills.
  • Lead the development of clinically effective, multidisciplinary team of practitioners to enable the implementation of effective admission avoidance initiatives
  • Support/ lead on development of integrated working an integrated pathways with Primary and Secondary care colleagues and frailty services
  • Be responsible for the development and delivery of a consistently high quality service, promoting frailty care as a positive place for staff experience and development.
  • Act as an expert practitioner and a role model to and on behalf of the whole team, to ensure greater integration of the frailty pathways into health & social care admission avoidance initiatives.
  • Working in partnership the post holder communicates effectively with patients and their carers, the primary health care team, Secondary Care, Care Home and voluntary sector and other key stakeholders on a range of complex health care matters, issues, ideas & situations

  • Ensures effective liaison and communication with other professional groups and stakeholders, acting as an information source and works in partnership in the interest of patients
  • Acts as an information resource for patients, multi-disciplinary colleagues and other stakeholders.
  • Maintain high visibility to patients, carers and other professionals including patients requiring long term condition managemen
  • Use advanced skills and expert knowledge to clinically lead the delivery of best practice across the health economy.
  • Keeps accurate, contemporaneous records and provide written reports, as required.
  • To lead service development projects and new ways of working.
  • To manage and prioritise workload to ensure deadlines are met and a high standard of service  is provided Evaluate care provided within teams and support to improve where required in terms of quality and efficiency to maximise cost effectivenes
  • Ensure optimum use of all resources within the teams and proactively ensure that services within the business unit are delivered on target and within budget. 
  • Ensure Risk Management strategies are effectively utilised within the team.
  • Ensure the service is delivered in line with all relevant legislation and statutory requirements as well as Trust policies, protocols and guidance. 
  • Contribute to the compliance of Care Quality Commission requirements, regulations and standards; and Trust quality improvement initiatives where appropriate.
  • Lead on delegated projects as required and requested Demonstrate the ability to function as a clinical leader.
  • Have the skills necessary to deliver both theoretical and practical clinical training on topics related to the service. Acknowledge and support the roles and functions of other team members
  • Undertake advanced physical assessment and history taking in order to reach diagnosis and management plan for healthcare problems and conditions
