We are seeking to recruit an experienced, competent and motivated Occupational Therapist to join our new specialist palliative care therapy team based at St John’s Hospice in Doncaster. St John's Hospice is an NHS Hospice. 

This role will cover specialist Occupational Therapy input for patients within the St John’s Hospice Inpatient Unit, the St John’s Hospice Day Therapy Unit and outreach within patient homes in the community.

This role will focus around embedding a rehabilitative palliative care model using a person-centred approach. Key elements of the role will include holistic Occupational Therapy assessments, equipment provision, discharge planning, activity advice, symptom self-management advice, and signposting for patients with a life-limiting, palliative condition. This may be completed on a 1:1 basis or within group sessions in the Day Therapy Unit. Experience and a keen interest in the palliative care speciality is essential for the successful candidate.

Main duties of the job

·         Perform holistic Occupational Therapy assessments with patients including  formulating individual treatment plans in conjunction with the patient in both the inpatient unit, day therapy unit and patient homes in the community.

·         Plan, co-ordinate, deliver and evaluate proactive therapeutic interventions to promote health and wellbeing, assisting patients to self-manage their medical condition.

·         Maintain a clinical caseload of patients as an autonomous practitioner and appropriately document any interventions completed. 

·         Communicate and collaborate with others involved in the care of your patients for excellent outcomes.

·         Provide or direct patients to appropriate equipment and/or minor adaptations when required. 

·         Participate in the supervision, support, teaching and appraisal of students, qualified staff and Therapy Support Workers.

·    Devise and delegate work to Therapy Support Workers dependent on level of competence. Educate and support them to achieve excellent patient outcomes. 

·         Work in partnership with colleagues within the service and external agencies to maintain on going service development and quality improvements.

Participate in maintaining and improving all aspects of evidence-based practice and quality assurance including change management.

·         To provide intervention on 1:1 basis or in group settings.

  • Diploma/Degree in Occupational Therapy
  • Registered with the Health Care Professions Council
Desirable criteria
  • To have completed the L’APPLE course
  • Post graduate courses associated with Palliative care and the end of life patient.
  • Management/Leadership qualification.
