Job overview

  • To provide support to the Service manager to meet the aims and objectives of Doncaster Care Group.
  • To provide support to the area service manager in the effective management of Registered General Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Assistants within the resources available.
  • Reviewing staffing levels and skill mix as necessary.        
  • To be responsible for the leadership, mentorship and support of staff on a daily basis
  • Manage and provide management supervision to Registered General Nurses, Physiotherapists and Occupational Therapists. Additionally to ensure clinical supervision for all other team members is in place.
  • Work within the infection control agenda to improve cleanliness
  • Improving environments for patients
  • Resolve problems for patients and their care/relative by building closer relationships
  • Building partnerships with other partner Organisations building community strength in localities.

Main duties of the job

•    Be responsible for the leadership, mentorship and support of staff on a daily basis.
•    Support the service manager in Service Provision in the effective management of caseload holders within the resources available, reviewing staffing levels and skill mix as necessary.         
•    Ensure that effective two-way communication takes place throughout the Care group, at all levels, by disseminating information.
•    Act as a professional link, under the supervision of the service manager between the Care group director and staff, canvassing opinions and correlating feedback from staff.
•    Be involved in Quality, Standards of Care and Clinical Audit within the Trust, including monitoring staff records.
•    Stimulate and participate in research, whilst supporting staff undertaking research.
•    Ensure the undertaking of the PDR process for staff to create a Personal development plan to identify training and development requirements.
•    Ensure the undertaking of the supervision process to ensure all staff get the support they need.
•    Lead the Home First Community Teams in Task Groups following consultation with the Service Managers and be involved in Subgroups, Working Panels and Working Groups as the need arises within the Trust to redesign and evolve services.
•    In consultation with the Service manager, the Team Leader will lead on annual objectives both joint and individual, to be reviewed on a regular basis to meet organisational, individual and professional needs.

  • CSP qualification in District Nursing or equivalent
  • Educated to degree level or evidence of able to work at this level
