Job overview

The Unplanned Community Nursing Team reflects an integrated approach that has been designed in response to the changing needs of the local population, with a special emphasis being placed on the proactive vision of care for patients in the community setting.  The Unplanned Community Nursing Team provide a 7 days a week service; 24 hours a day, which can react to the patients needs as and when required. 
As the unplanned care team, we cover the whole borough of Doncaster and provide all aspects of nursing care to patients in their own homes. We offer a supportive environment with opportunities for further career development, a strong in-house training programme, including excellent links with our partner Universities and opportunities to gain further qualifications. We offer a comprehensive 12-month preceptorship for all  staff new to post.
We believe in care that treats the whole person, not just the condition. We enable patients to stay at home longer and leave hospitals faster. Our community nurses work closely with members of the wider multi-disciplinary teams, including our GP's, local hospice, Clinical Lead Nurses, and specialist nurses.
We are seeking to appoint a Staff Nurse who can demonstrate the vision and skills to work innovatively, flexibly and in partnership with other agencies supporting the MDT team assessment and provision of care to the high standard we aim for.

Main duties of the job

This role not only involves assessing, planning, implementing and evaluating  nursing care in the patients own home but also over the telephone. The post  holder will have the ability to utilise excellent communication skills, IT skills, clinical expertise, telephonic clinical assessment, diplomacy skills and judgement to ensure that referrals into Community Nursing are managed effectively and the relevant services are involved within appropriate timeframes. 

  •  Setting, maintaining and delivering high standards of care in a variety of locations.
  • Under the supervision of a Senior Nurse, triage incoming referrals assessing the patient's needs and  determining the priority of care  accordingly.
  • A multi system’s approach to case management, linking in with case managers and the wider health and social care community, to ensure a seamless patient journey for a wide variety of care needs across the Doncaster area.
  • The first line management of junior staff within the team, offering support, mentorship, acting as a positive role model, having a solution  focused ‘can do’ approach to any challenge, ensuring that standards are maintained and the best practice evidence base is adhered to.
