Job overview
To work in collaboration with other colleagues towards open and easy access to acute primary health services for people with a learning disability. The philosophy outlined in ‘Valuing People Now’ and embedded in the strategic Health Facilitation Framework will provide the overall structure for this work.
Main duties of the job
1 To identify training and education needs of people with learning disabilities and their families and carers in relation to supporting individuals with promoting their own health and the proactive use and access of mainstream health services, including the specific needs of the ageing population with learning disabilities.
2 To work as part of a team but also be competent and confident to work independently, providing expert advice to all allied health professionals.
3 Provide professional support and advice to colleagues and senior managers within the organisation.
4 Ensure that the standards and recommendations identified in Valuing People and Valuing People Now, National Service Frameworks, Disability Equality Schemes and the enquiry report Death by Indifference, are routinely applied to the care of people with learning disabilities, and in particular the ageing population with learning disabilities.
5 Support the development of health facilitation, reflecting local user’s views, national best practice approaches and local demographic needs. Implementing and co-ordinating publicity, training and advice on this development.
6 Implement Health Action Plans and hospital passports for people with learning disabilities, with a Doncaster GP.
7 To provide an evidence based approach to the effective delivery of health facilitation, which aims to inform, educate and support, along with advice on training to all key stakeholders.

Qualifications
Essential criteria
- RNLD
- ENB 998 or equivalent
- Evidence of on-going continuous professional development
Experience/ Knowledge
Essential criteria
- Experience in a variety of care and support settings for people with learning disabilities
- Ability to work with families and carers
Desirable criteria
- A minimum of 1 years post registration experience
Applicant requirements
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
