Job overview

As a key member of the Gender Identity service’s Senior Leadership Team and working under the professional direction of the Head of Nursing the Lead Nurse is responsible for the implementation of National, and corresponding local Nursing Strategy.

The Lead Nurse is accountable for developing a culture of clinical improvement, leading by example and empowering staff to improve the care and well-being of service users in their clinical area. This means being known and visible to staff and being familiar with the service users, their needs and the challenges faced by your staff.

The Lead Nurse holds local accountability for trust wide nursing initiatives such as Infection Control, Physical Health and workforce health and safety. SHSC view the Lead Nurse as a clinical leader who exercises great influence over junior staff and has the attributes to promote a culture which nurtures practice excellence and development for all those in the designated area of practice.

Main duties of the job

  •  To have knowledge and skills pertaining to the relevant specialist area.
  • To support coordination of the Gender Identity Clinic and build effective relationships with key stakeholders.
  • To provide the highest quality services within the resources allocated, ensuring people accessing the service are offered appropriate high-quality care to meet all of their care needs.
  • To work with local stakeholders to design and deliver safe and effective care pathways for people accessing gender identity services.
  •  To function as a named professional for a defined group of service users, as defined in the service specification, and to lead the other team members also providing named professional roles to clients.
  •  Maintaining own clinical competence and active NMC registration ensuring at least 20% of role is undertaken through clinical activity, acting as a role model.
  • To provide strong, effective and decisive clinical leadership to the multi-disciplinary team to ensure a high standards of patient care are achieved.
  • To line manage a small team of practitioners within the service.
  • To work with the MDT to support identification of clinical risks for patients and for the service and implement improved risk management and care plans and systems where necessary.
  • To work with the MDT ensuring that patients are treated with respect by supporting the protection of their privacy and dignity and making sure they are addressed in the way they choose.

Detailed job description and main responsibilities

 Key Responsibilities 

Clinical leadership

•    To support coordination of the Gender Identity Clinic and build effective relationships with key stakeholders. 
•    To provide the highest quality services within the resources allocated, ensuring people accessing the service are offered appropriate high-quality care to meet all of their care needs.
•    To work with local stakeholders to design and deliver safe and effective care pathways for people accessing gender identity services. 
•    To ensure that services are delivered within a robust clinical governance framework, including monitoring, and evaluating standards of care routinely in accordance with trust and NHS England governance arrangements.
•    You will embed the standards of care detailed in the national service specification, evidenced via audits and surveys, including the CQC standards, seeking to attain outstanding in all areas. 
•    To create and sustain effective working relationships and clinical pathways with key stakeholders such as primary care and surgical providers. 
•    To maintain effective working relationships with relevant voluntary, and independent sector organisations.
•    To work closely with the rest of the leadership and wider team. 
•    To function as a named professional for a defined group of service users, as defined in the service specification, and to lead the other team members also providing named professional roles to clients.
•    Maintaining own clinical competence and active NMC registration ensuring at least 20% of role is undertaken through clinical activity, acting as a role model.
•    Working in partnership with other clinicians and leaders across SHSC to ensure a collaborative approach to managing resources.
•    To have knowledge and skills pertaining to the relevant specialist area.
•    To co-lead on the regular and systematic evaluation of care standards via formal audit or other ensuring that any necessary changes are actioned. 
•    To provide strong, effective and decisive clinical leadership to the multi-disciplinary team to ensure a high standards of patient care are achieved.
•    To contribute clinical leadership to relevant nursing and leadership systems, which may include contribution to a leadership on-call system.
•    To contribute to Trust wide nursing strategy formulation, and to work in conjunction with the Head of Nursing in the implementation of the strategy locally, monitoring and evaluating progress.
•    To provide clinical supervision to senior nurses within the directorate, ensuring that standards of nursing advice are consistent. 
•    Lead nursing developments in accordance with the Trust Nursing strategy and Trust / Directorate objectives using quality improvement methodology.
•    To lead on the development and implementation of effective systems for monitoring and improving the quality of named professional care.
•    To act as a clinical advisor and role model for staff, applying problem solving skills and offering support where possible.
•    To lead on the development and implementation of new policies and protocols and review practices in relation to new relevant practice guidance.


Managerial 

•    To line manage a small team of practitioners within the service. 
•    To work with the MDT to support identification of clinical risks for patients and for the service and implement improved risk management and care plans and systems where necessary.
•    To undertake specific service improvement and development projects, as agreed by the General Manager, Head of Nursing and Head of Service using quality improvement methodology.
•    Ensuring people are safeguarded from harm and that all clinicians in the team understand their responsibilities to safeguard all in their care.
•    Listen to staff feedback regarding work-based needs, working collaboratively to develop local solutions escalating risks to Head of nursing.
•    Identify gaps in skills of staff and ensure support and development as required.
•    Application of audit processes to assess performance against standards of care (including documentation).
•    Support and take part in quality improvement projects.
•    To monitor team and staff performance via the use and analysis of dashboards and other relevant data.
•    To implement action plans resulting from investigations / enquiries and other processes, and ensure learning is embedded.

Education and empowerment 

•    To work with the Head of Nursing and MDT to ensure that practice reflects current clinical developments and is evidenced based.
•    Implement audit and conduct competency assessments of clinical staff.
•    To provide support and direct input to new staff.
•    Ensure team managers enable effective student nurse placement and facilitation of other learners to meet placement learning requirements. 
•    Support the rollout and implementation of key nursing agendas. 
•    Have a central role in assimilating national research and guidance and identifying service needs and gaps.
•    To support relevant local research and service evaluations and implement learning from findings and recommendations.

Safe environments 

•    To work in partnership with the Trust Infection Control Team to ensure that clinical areas meet the required standards.
•    To ensure the clinical environment meets the requirements and needs of the client group. 
•    To undertake audits (e.g. health and safety, care standards) and implement action plans as required. 
•    To work together with other professionals to develop an organisational culture that ensures the clinical environment is fit for purpose.

Service feedback, complaints, and investigations 

•    Following up on incidents reported, highlighting, and addressing gaps in practice.
•    Contribute to complaints and investigations as required or directed by the Head of Nursing. Work with the Head of Nursing to identify specific trends regarding incidents and lessons learnt.
•    To be visible and accessible to patients, relatives, and staff to provide assistance, advice and support, this also includes liaison with the complaints service. 
•    To lead on service user engagement and feedback mechanisms, ensuring the service learns from these processes. 
•    Encourage and support co-production as part of the service management team.
•    Support the development of systems to support relatives and carers.
•    As required, to deal promptly with concerns raised by patients, carers / relatives, and staff. 
•    Work sensitively where dissatisfaction is expressed and resolve concerns locally as far as possible.
•    Ensure that the findings of investigations are considered, and action is taken locally to learn from incidents and complaints.

Risk management

Staff should be aware of the principles of risk management and adopt
appropriate practice to reduce the risk to themselves and others. In particular,
staff should observe the Trust’s Risk Management Strategy, Risk, Incident &
Serious Incident reporting and management policies, Health & Safety
regulations and the risk management training requirements for all staff.

Financial Responsibility

To work with the senior manager to achieve financial targets including the Cost Improvement Programme.

To ensure compliance with all Trust Policies and Procedures supporting robust financial management.

Human Resources

To maintain the team performance management framework, taking appropriate actions, and ensuring that there is clear communication of the team functions, standards, and targets.

To manage team performance to ensure team targets are achieved.

To ensure that all staff participate in appraisal, with an appropriate plan for training and development that links to team functions and targets.

To work with professional leads to ensure that all team members receive appropriate supervision and that professional standards are maintained.

To work with professional leads to establish an effective recruitment process.

Training & Development

To participate in regular supervision in accordance with good practice guidelines and Trust policy and participate in the Trust’s annual Appraisal process.

To attend all relevant mandatory training as and when required to do so and take opportunities to extend develop clinical expertise and appropriate management skills, within the aims of the service and available funding, and maintain a professional portfolio of experiences.

Adhere to and promote adherence to the NMC code of conduct, providing evidence of relevant registration as required.

Work closely with the senior nurses within the Trust to identify the training needs and deficits in the accountable workforce agreeing how these will be met, including agreeing specific priorities arising from clinical events or new legislation, ensure that a learning environment and robust preceptorship programme is established in the practice area.

  • Post graduate qualification or demonstrable equivalent in a clinical/leadership area with relevance to a gender identity clinic MDT

  • Non-medical prescribing qualification relevant to gender identity services
