Job overview
The Memory Service offers an exciting opportunity to work with older adults who are experiencing memory difficulties. The post holder will work closely with the service user and their supporters/next of kin/carers to ensure that their experience from assessment, through to diagnosis and post diagnostic planning, ensuring that their care is personalised with them at the centre. If you have an enthusiastic and positive approach to working with people with dementia and like to work in a supportive multi disciplinary team this may be your opportunity
Main duties of the job
The post holder will work within a multi-disciplinary service
- assess, plan, implement, evaluate treatment and collaborative care planning
- will provide cognitive testing and assessment in the community or hub base
- deliver group/1:1 education interventions
- will maintain and promote a personalised approach in all aspects of service delivery to service users and their significant others using our service.

Detailed job description and main responsibilities
- To provide cognitive testing, assessment, diagnosis and review in the community/hub
- Coordinate and actively participate in the daily running of the Case Management Team in the absence of the substantive post holder
- Work collaboratively with peers to co-ordinate the duties and responsibilities of other nursing and support staff within the service on a day to day basis
- Actively contribute to the development and delivery of therapeutic/treatment plans and psychosocial/educational interventions
- Ensure, through supervision, that each allocated service user has a comprehensive, clear and up to date collaborative care plan with specified objectives designed to meet the individuals identified care needs, ensuring effective clear communications and service user/significant others involvement in the formulation, while respecting the wishes and autonomy of the individual
- Participate and input into the multi-disciplinary team meetings [MDT] providing follow up as necessary/indicated post MDT
- Identify and make appropriate referrals to other agencies/services to support service users needs
- Provide support and guidance to junior staff members
- Develop and maintain effective working relationships with Primary Care colleagues and 3rd party sector
- Participate with colleagues in the promotion of an active learning environment in order to maintain and develop high standards of professional and clinical practice. Promoting opportunities for self, nursing colleagues and support staff to access available education/training.
Person specification
Knowledge and Skills
Essential criteria
- Registered Nurse Mental Health or Occupational Therapist, 2 years post grad
- Working knowledge and understanding of older adults with organic mental health
Desirable criteria
- Non medical prescriber or willing to undertake training
- Evidence of delivering a group intervention
Training and Qualifications
Essential criteria
- Registered Nurse Level 3 (RMHN) or Qualified Occupational Therapist
- Mentor or assessor Qualification or equivolent
- Evidence of continued professional development relevant to role
Desirable criteria
- Use of assesment tools e.g. Addenbrookes Cognitive Exam lll; Geriatric depression scale
- Diagnosis feedback
Experience
Essential criteria
- Leadership and interpersonal qualities
- Effective time management
- Ability to demonstrate sound knowledge and application of up to date evidence based clinical practice in dementia care
- Use of a car
