Job overview
• The post holder will function as an autonomous practitioner working in conjunction with other members of the Gender Identity Clinic as part of a multidisciplinary team.
• The post holder will undertake comprehensive assessments, manage a complex caseload of service users, provide advice, support, deliver interventions and respond to any adult safeguarding issues. They will also monitor medication management and concordance and monitor effects under supervision of a lead clinician.
• To work to develop and deliver safe and effective services, in accordance with key national targets, clinical standards, service priorities, and performance indicators set by regional and national commissioners and Trust and service policies.
• To provide specialist health advice, direction, and guidance to service users, colleagues and other stakeholders.
• Work in partnership with commissioners, regional service colleagues and other partner agencies to provide gender identity services that achieve the best possible outcomes for service users.
• To offer line management to members of the clinical team where required in collaboration with other service leaders.
Main duties of the job
• Work as part of a multi-disciplinary service fostering good communication with colleagues of all disciplines whilst ensuring that all communications are conducted with dignity and respect.
• Establish rapport and maintain supportive professional relationships with patients and their carers, supporting them through the assessment process even when there are barriers to acceptance such as lack of insight or difficulties adjusting to diagnoses and complex life changes.
• Be responsible and accountable for receiving and providing complex, contentious, and sensitive information to patients regarding their health status or future treatment.
• Take and accurately record patient histories and medical conditions, including physical health.
• Advise and take informed consent for investigations and clinical procedures and disclosure of sensitive personal information.
• Communicate contentious information in unpredictable, confrontational / volatile situations where clients, carers, staff may be stressed / distressed, abusive and anxious / irrational.
• Maintain effective communication with colleagues in health and partnership agencies, negotiating and agreeing individual roles and responsibilities in relation to the delivery of care, respecting user choice and autonomy throughout the process.
• Act as the Named Professional as defined by appendix G of the Service specification: Gender Identity Services for Adults (Non-Surgical Interventions)* for an agreed caseload.

Detailed job description and main responsibilities
- To develop the service in line with local and national directives ensuring service delivery follows agreed pathways and evidence-based practice.
- To lead the development and implementation of clinical guidelines and protocols relevant to the Gender Identity Clinic in partnership with relevant colleagues.
- To provide expert advice and support to colleagues and other agencies involved with the provision of care for those experiencing the challenges of Gender Dysphoria contributing directly to the formulation and treatment plan.
- To be responsible for implementing a range of specialist evidence-based interventions for individuals, carers, families and groups within and across teams.
- To maintain accurate records and manage data collection systems to assist in the evaluation and development of the service.
- To coordinate and deliver education and training to build confidence, knowledge and skills of all stakeholders, enabling them to better support gender diverse people
Person specification
Knowledge
Essential criteria
- Substantial experience at b6 or above in the care of complex presentations in an NHS setting
Desirable criteria
- Non medical prescriber qualified
Experience
Essential criteria
- Demonstrable evidence of knowledge of transgender health care issues
- Worked in a NHS community MDT/ outpatient facility with patients with complex needs
Desirable criteria
- Previous experience of working in an adult or CYP gender identity team
Skills
Essential criteria
- Demonstrable experience of working in environments where it is necessary to communicate contentious information in unpredictable, confrontational / volatile situations where clients, carers, staff may be stressed / distressed, abusive and anxious / irrational.
