Specialist Nurse Looked After Children

How would you like to make a difference to the outcomes of children in care within a dedicated, well established team?
Do you have experience of working with children in care?
We are looking to recruit motivated practitioners with initiative and drive to join our friendly and enthusiastic team and contribute to the further development of our service.
We support approximately 2500 children in care across the areas of Staffordshire and Stoke on Trent. We undertake the statutory health assessments for these young people, ensuring that they remain the focus of the assessment and that their health needs are met.
The role will require travelling across the geographical area of Staffordshire and at times slightly beyond.
Prif ddyletswyddau'r swydd
- Undertake health assessments for looked after children and complete summaries and health care plans with SMART action plan and provide written/electronic reports as required.
- Following national and local guidelines, plan, organise, deliver and evaluate comprehensive health assessments to assess and formulate the complex needs of a looked after child or young person using evidence based knowledge to develop health plans toward improving the long term health outcomes for looked after children
- The post holder needs to be an autonomous practitioner who develops and embraces a flexible range of working regarding the way services can be delivered to children and young people who are looked after toward achieving positive outcomes. Post holder to be available for the occasional evening work.
- Ensure that the health of Looked After Children is positively promoted, encouraged and supported. Empower children and young people to take responsibility for their own health and wellbeing

Experience

- Post-qualifying experience in health visiting or school nursing, with experience of the health needs of Looked After Children.
- experience of working with children and young people with emotional health issues.

Skills

- Excellent IT skills, ability to use Microsoft Outlook, Word
- Excellent written and verbal communication skills.
- Knowledge of health inequalities faced by children in care
