Job overview
We have an opportunity for a highly motivated and autonomous Specialist Occupational Therapist to join our well established Discharge to Assess (D2A) pathway 2 Service. Following successful service development, we are developing our D2A pathway 2 service giving you the opportunity to help drive developing and implementing high quality care pathways and MDT training plans. Actively contributing to building a culture of research and development, with strong links to our local community supported by an invested interest in improving our bed-based services.
Main duties of the job
This is a unique opportunity to work in a transformative and innovative way with the autonomy to shape the future of D2A within North Staffs. Working in co-production with our service users and local D2A networks; with the overall aim being to improve the health and wellbeing of the population served.

Detailed job description and main responsibilities
We offer excellent support through CPD routes and building robust learning plans within the four pillars of AHP practice; offering opportunities to represent and promote our service locally, regionally, and nationally. Our service is transformational with the post holder requiring a level of NHS experience and confidence to support with helping us drive development of the next exciting phase of our service model. The community role will require access to a vehicle, with UK driving licence and ability to work across a wide geographical area.

Qualifications
Essential criteria
- • Degree in Occupational Therapist or equivalent. • Registration with HCPC • Evidence of postgraduate CPD in at least 2 relevant areas
Desirable criteria
- • Modules at Masters level • Evidence of further clinical study to D2A / adults / rehabilitation • Falls training & related assessments • Mental capacity training • Member of RCOT • Member of relevant clinical interest group
Experience
Essential criteria
- • Relevant experience to the role in a variety of specialties including neurology, orthopaedics, older peoples rehabilitation, falls management, respiratory care, musculoskeletal including assessment and management of various conditions within the area of clinical speciality.
Desirable criteria
- • Experience of risk management including risk assessment, action planning & evaluation
