Job overview

The local ICS healthy ageing and frailty strategy recognises that new models of care in the community are required to manage the increasing demand on services driven by our ageing population.

 

Midlands Partnership NHS Foundation Trust has ambitious aspirations to establish a model of care which supports the development of a centre of excellence for holistic care in rehabilitation and recovery and provide medical leadership for frailty across our community services.

 

You will be a figurehead of the services based at Haywood Hospital and will alongside a multi-professional team provide input across the bed based Rehabilitation and Assessment Service.

 

The role will provide clinical leadership, mentoring and support to the team delivering the Rehabilitation and Assessment Services.

 

 

Main duties of the job

  • Provide clinical and subject matter expertise support to the MDT, advising, mentoring and supporting complex clinical decision making.
  • Contribute to / leading Board / Ward rounds, MDT’s (multi-disciplinary team meetings), MADEs (multi-agency discharge events) and long stay reviews across the Discharge to Assess pathway (Haywood Hospital, CCG Commissioned Beds, Home First)
  • Provide leadership support to service improvement and ongoing system wide development
  • Teaching
  • Research and audit (links to local academic organisations)
  • Supporting End of Life pathways and ongoing improvement through established system portfolio working groups

  • To provide leadership for a team that delivers high quality medical care to patients on our D2A pathways, which is timely and inclusive. Supporting people to make the right choices to suit their individual circumstances and preferences.
  • To become a service of excellence through supporting teaching, training and research
  • To provide consistency and continuity of care for Haywood Hospital patients in and out of hours
  • To continually improve processes and pathways across all providers involved in the D2A pathways.
  • To work in collaboration with other Trust clinicians including non-medical workforce.

Person specification

Qualifications

Essential criteria
  • Full GMC registration
  • At least 6 years UK experience in Geriatric Medicine
Desirable criteria
  • Diploma in Geriatric Medicine
  • MRCP or equivalent

Skills

Essential criteria
  • Demonstrates the underpinning subject specific competencies i.e. knowledge, skills and behaviours relevant to the role setting and scope.
