Are you an experienced Community Mental Health Support Worker looking for a new opportunity to become a Rapid Responder and work in an organisation that will help you develop and achieve your goals to become the best you can be? If so, you're in luck. 

 

Midlands Partnership University NHS Foundation Trust has an opportunity to join our new Adult ADHD/ASC Service. The role will be based in South East Staffordshire, covering Lichfield, Tamworth and Burton on Trent. 

 

MPFT are pleased to announce we will be providing assessments for ADHD and Autism for adults in South Staffordshire. We will be having two parts to the service, assessment and post diagnosis support. We will have a multi-disciplinary team including Consultant Psychiatrist nurses, occupational therapists, peer support workers and more. We strive to meet the needs of our locality providing a gold standard service.

•    Identify risks through use of appropriate assessments, act immediately if the situation requires, and ensure all risks are reported to senior clinicians/line managers.
•    Implement and support detailed care plans, outcome measures documentation, risk assessments and treatment programmes (including intervention Support Plans, mental health and multi-disciplinary care plans)
•    Review the ‘Duty Inbox’ for the team and signpost service users accordingly 
•    Offer interventions in line with ‘Overcoming Challenging Times Plan’
•    You will work without direct supervision to undertake assessments and deliver interventions relating to individual needs.
•    Support and assess patients on point of referral under supervision. 
•    Working collaboratively with other Trust services to provide seamless care for service users in the Integrated Mental Health Teams

The role of the rapid responder is to provide support in bridging the gap between the service user and their next clinical contact, addressing any unplanned service user requests.
You will have the opportunity to work as a member of the multi-disciplinary team, which includes other allied health professionals, nursing, medics, social workers, and psychology. Your role will involve working closely with other agencies, carers, service users, families, the local community, and Community Mental Health Teams, ensuring continuity of service is provided at a high level.

  • Foundation degree in relevant subject or Diploma/NVQ 3 in Health and Social Care OR accredited prior learning with competency based evidence supplemented by short courses.

  • Training in working with adults / older adults with complex mental health needs and/or challenging behaviour, and/or other behavioural work

Experience

  • Significant experience of working with adults / older adults with Mental Health Needs as a member of a multi-disciplinary community team Experience of working within a multi-agency framework
  • Experience of formulating a service users difficulties and developing a care plan
  • Experience of employing evidenced based interventions.
  • Experience of working with families and carers
  • Able to work independently, without direct supervision
  • Experience of supporting other workers and role modelling recommended approaches / interventions in practice.

  • Previous work with people with complex and high risk presentations
  • Experience of supporting service users in a home based/ community setting
  • Experience of working with ADHD/ASC
