You will support the qualified clinicians in the delivery of person-centred care based upon the person’s negotiated goals and relevant evidence-based interventions.  In addition you will support qualified staff with coordination of assessments within the Health Based Place of Safety when required.

You will be responsible for the provision of planned recovery focused care working in partnership with service users. You will work collaboratively with other Trust services to provide seamless care for service users on defined care-pathways.

Main duties of the job

 

  • Ensure the delivery of high quality care through high personal standards in clinical practice.
  • To ensure care is provided in line with the associated NICE Guidance and other relevant documentation and that there is knowledge of national, regional and local drivers underpinning service delivery including the Mental Health Act 1983 (revised 2007) regulations & the Mental Capacity Act (2005).
  • See service users as appropriate, to carry out agreed interventions under the supervision and guidance of the lead professional.
  • Work with multi-disciplinary team registered nurses to meet the daily living needs of service users
  • To participate where appropriate in robust risk assessments and risk management plans under the supervision and guidance of the multi-disciplinary team and support the team to practice positive risk taking where appropriate to improve quality of life, well-being and maximise occupational performance/independence.
  • Under the supervision and guidance of the multi-disciplinary team to deliver a range of non-pharmacological interventions following specific assessments to enable understanding of any expression of an unmet need and to maximise an individuals abilities.
  • Accurately undertake and record patient observations, under the guidance of a registered nurse.
