The Acute Outreach team offer a responsive service to avoid unnecessary hospital admissions, other delayed transfers of care and facilitate hospital discharges. To support the care of service users and carers out of hours, to ensure timely interventions to support early treatment and reduce the risk of carer breakdown. The Acute Outreach team ensure an integrated approach to the care of people with dementia and functional mental health conditions. They work in partnership with local health, social care, and voluntary organisations.

Prif ddyletswyddau'r swydd

•        To contribute to the assessment, planning, implementation and evaluation of the evidence-based interventions required, for a named service user/group under the guidance of a senior practitioner.
• To provide professional advice and support to other agencies and members of the multidisciplinary team.
• To be responsible for the appropriate clinical risk assessment of service users. Providing effective advice, guidance and decision-making process culminating in the coordination of any reviewed plan of care under the supervision of a senior practitioner.
