Band 6 School Nurse- Children and Young People, Targeted Intervention Service.

The School Nursing Targeted Intervention Service for Children and Young people age 5-19 years old, in Stoke on Trent, have a permanent vacancy within our enthusiastic, caring and supportive team. We are looking for a School Nurse who lives the values and behaviours of our Trust.

We are seeking a person who will be caring and compassionate providing excellent support to children and their families, having the ability to actively listen and have a warm approachable manner. You will lead by example and encourage effective team working, someone who wants to develop others and ensure the best outcomes for our children and families are achieved through evidence-based practice. In return you will be professionally supported and empowered to deliver a child friendly service. We are a friendly team and we welcome an experienced or newly qualified School Nurse to join our diverse team where flexible working is supported and you will have a comprehensive induction programme.

The successful applicant must have the ability to travel within the Trust and be available for delivering the service between the hours of 9am to 5pm Monday to Friday.  The post is for 30.00 hours per week which can be flexible to meet the needs of our service and clients.

Main duties of the job

The post holder will be responsible for the leading and delivery of the School Nursing Targeted Intervention Service and any related Service Specifications within their geographical area.  In addition, the post holder will be expected to supervise and delegate activities and responsibilities to other members of their team.

The post holder will be responsible for the area as an autonomous practitioner. In doing so, they will assess, plan, deliver and evaluate patient centred health care to a specialist level ensuring that they and their team practice within their sphere of competence and knowledge. The post holder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.

Detailed job description and main responsibilities

To lead the provision of expert nursing advice, assessment, care planning and support to children and young people identified as having an additional health need.

Co-ordinate the appropriate allocation of casework and targeted intervention within the geographical area

Utilise key skills of assessment, interpretation and analysis of findings, utilising clinical judgements.

Assess, plan, co-ordinate and evaluate care for children and young people

Promote effective working relationships with the school setting to ensure children and young people are able to access education as their condition allows

Actively support children, families and carers to self-manage their conditions by advocating independence

Maintain a non-judgemental approach regardless of circumstances when working within the community and in all aspects of his/her work

Teach children, young people and carers to competently carry out specific nursing procedures, maintain contemporaneous and accurate records of care.

Use knowledge and expertise to support other professionals on a range of health conditions, act at all times sensitively to the differing social, cultural and religious needs and wishes of each child and their family.

Support education and training settings (including independent providers) to fulfil their welfare role by offering additional training on “condition management” where this does not fall as part of the “universal offer” within the Public Health commissioned Specification.

To plan negotiate and carry out initial and any subsequent on-going child health assessments, using relevant policies, pathways, procedures and guidelines; and make appropriate referrals to other health and multi-agency partners as required

To participate in multi-agency decision making, including assessments (where required) in line with Safeguarding policies and procedures to ensure the best outcomes for children and young people.

To contribute to the development and implementation of policies across the Trust reflecting service developments and improvements.

Demonstrates a detailed theoretical and practical knowledge of common health conditions and presentations – involving a critical understanding of theories and principles. Developing, implementing and evaluating individual care plans for children with specific health needs, contributing to Education, Health and Care Plans where appropriate.

Demonstrates innovative and specialist skills to select methods and tools to holistically assess, plan, implement and evaluate in a complex and specialised field of work.

Makes consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensures clinical reasoning is reflected accurately in case records of their own and team members.

Engages and actively involving the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of care to promote a culture of involvement and empowerment

Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a competent standard

Prioritises their own workload, and that of the wider team with the support of the Team Leader and caseload work allocation.

Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all areas of practice including the management of any complaints and compliments.

Essential criteria
  • RGN, RN, RSCN or RN child branch
  • safeguarding children level 1 and level 2
Desirable criteria
  • Specialist Community Public Health Nurse (SCPHN) - school nursing
  • Mentorship/teaching qualification

Experience

  • experience in school nursing

Desirable criteria
  • experience working with children with complex needs.
