To lead the provision of expert nursing advice, assessment, care planning and support to children and young people identified as having an additional health need.

Co-ordinate the appropriate allocation of casework and targeted intervention within the geographical area

Utilise key skills of assessment, interpretation and analysis of findings, utilising clinical judgements.

Assess, plan, co-ordinate and evaluate care for children and young people

Promote effective working relationships with the school setting to ensure children and young people are able to access education as their condition allows

Actively support children, families and carers to self-manage their conditions by advocating independence

Maintain a non-judgemental approach regardless of circumstances when working within the community and in all aspects of his/her work

Teach children, young people and carers to competently carry out specific nursing procedures, maintain contemporaneous and accurate records of care.

Use knowledge and expertise to support other professionals on a range of health conditions, act at all times sensitively to the differing social, cultural and religious needs and wishes of each child and their family.

Support education and training settings (including independent providers) to fulfil their welfare role by offering additional training on “condition management” where this does not fall as part of the “universal offer” within the Public Health commissioned Specification.

To plan negotiate and carry out initial and any subsequent on-going child health assessments, using relevant policies, pathways, procedures and guidelines; and make appropriate referrals to other health and multi-agency partners as required

To participate in multi-agency decision making, including assessments (where required) in line with Safeguarding policies and procedures to ensure the best outcomes for children and young people.

To contribute to the development and implementation of policies across the Trust reflecting service developments and improvements.

Demonstrates a detailed theoretical and practical knowledge of common health conditions and presentations – involving a critical understanding of theories and principles. Developing, implementing and evaluating individual care plans for children with specific health needs, contributing to Education, Health and Care Plans where appropriate.

Demonstrates innovative and specialist skills to select methods and tools to holistically assess, plan, implement and evaluate in a complex and specialised field of work.

Makes consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensures clinical reasoning is reflected accurately in case records of their own and team members.

Engages and actively involving the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of care to promote a culture of involvement and empowerment

Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a competent standard

Prioritises their own workload, and that of the wider team with the support of the Team Leader and caseload work allocation.

Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all areas of practice including the management of any complaints and compliments.
